Use of Advanced Preoperative Imaging in Classification of a Uterine Remnant

A 25-year-old presented with worsening left sided pelvic pain and severe dysmenorrhea. Two-dimensional pelvic ultrasound showed a suspected left adnexal mass. A pelvic MRI was obtained revealing a right unicornuate uterus with a suspected distal left uterine remnant with a 1.2 mm endometrial lining (Fig 1) while an additional three-dimensional transvaginal ultrasound (3D-TVUS) obtained confirmed functional endometrium with a left distal remnant (Fig 2). The patient underwent a robotic-assisted removal of the left uterine remnant and evaluation for endometriosis.
