An Electronic Health Record-integrated Web Application Augments a QI-Directed Morbidity & Mortality Conference and Improves Quality of Care

Precis: Using a novel web app to store and access M&M data in real time, we have demonstrated that the implementation of a QI-directed M&M format reduced system failure occurrence over time and improved timely completion of action items developed to address failures.
