Specialising in the US - setting up to minimise tax

In other countries, if you work as a dentist as an independent contractor, you can control when you pay yourself. So, say you earned 300k in a year before specializing, and then went and did a 3 year specialty where you won't make any money. You could use your company to pay you 100k a year over the 3 years.
