Preface

This is both a preface and a foreword. Why? Because, as within a preface, I will speak plainly about the purpose and creation of this issue, how and why this story came into being, and what the main points are going forward. But, as within a foreword, I should tell the reader why he or she should read the issue along with its potential importance in the field. So, bear with me. From the very beginning this whole journey got started a long time ago for me when my mentor, Nathan Zvaifler, invited his senior colleagues in the field of Rheumatology to UCSD starting in the 1970s with the purpose of enriching the scholarly and academic experience of his Fellows and colleagues.
