Causes of Vagally Mediated Paroxysmal Atrioventricular Block

To the Editor In a recent issue of JAMA Internal Medicine, Dr Zhou and colleagues presented an interesting and well-documented case of a patient whose Holter monitor revealed a 23.8-second ventricular asystole caused by paroxysmal atrioventricular block (PAVB). The authors convincingly demonstrated that the PAVB was the result of transient increase in parasympathetic tone rather than an intrinsic conduction system disease. However, the clinical cause of the vagally mediated PAVB and asystole was not explored or discussed.
