Urethra complications continues to remain a major concern for patients receiving ultrasound-guided high-dose-rate (HDR) prostate brachytherapy. One way to effectively minimize the incidence would be to avoid high doses in the organ, but this is difficult to achieve while ensuring adequate target coverage. Solutions with intensity modulated brachytherapy (IMBT) design concept, such as rotating shield brachytherapy (RSBT), has achieved this ideal dosimetry on-paper but its clinical implementation is challenging due to moving mechanisms that require high precision and the use of non-conventional HDR sources such as 153Gd, electronic, and 169Yb.
