We greatly appreciate the comment by Carlson and Hew,1 “Underwater versus conventional EMR of large nonpedunculated colorectal lesions: no clarity on the role of submucosal injection,” because it brings out the issue of combining strategies within underwater EMR (UEMR). As these authors say, Binmoeller et al2 described the UEMR technique as a resec tion strategy in which the floating effect allows the creation of a resection plane in the submucosal layer, without the need for submucosal injection.
