Information Needs and Design Requirements for an Application Supporting Safe Transitions Into Skilled Nursing Facilities

For older adults, discharges to skilled nursing facilities (SNFs) are the outcome of 20% hospitalizations1 and 13% of emergency department (ED) visits.2 SNF clinical staff rely on information from these settings to provide quality care and services to patients after these common transitions in care.3 Unfortunately, SNF staff have difficulty obtaining relevant and quality information from acute care settings.4 Additionally, when available to SNF staff, information is not organized to support decision making.
