how can i join foundation years in uk

hello I am a medical student in my last year in Sudan . I wander How is it going to be after graduation..... i want to join the foundation program in UK and i don't know how to do that ,,, my final exams is on 14-2-2016 and my university is listed on the World health organisation directory of medical schools in Sudan so i want to start my career there anyone can help me ?? and how to setup my plan ,,,
