Single‐lumen tunneled catheter: An old but useful option

We report a case of long‐term uneventful catheter use in a patient with previous recurrent vascular access dysfunction and infection. A single‐lumen tunneled catheter was inserted into the left internal jugular vein after a failed attempt of dual‐lumen permanent catheter placement. The follow‐up since device implantation has exceeded 5 years without any complications related to vascular access.

CONCLUSION: The hemodiafiltration with endogenous infusion system is a new dialysis technique that combines diffusion, convection, and adsorption processes. It allows for extensive solute removal, including protein-bound uremic toxins and some pro-inflammatory cytokines, but does not cause nutrient loss and inflammatory response during the treatment. Although the effect after a single hemodiafiltration with endogenous infusion session is limited, it may be improved by repeated and long-term treatment. AbstractBackgroundPlacement of covered self-expandable metallic stent (CSEMS) for post-endoscopic sphincterotomy (ES) bleeding achieves excellent hemostasis results. Although CSEMS placement is typically performed after failure of conventional endoscopic combination therapy, its excellent outcomes may justify earlier placement.AimsWe aimed to examine the efficacy of “early” CSEMS placement for massive post-ES bleeding.MethodsThe medical records of 2750 patients who underwent ES between 2005 and 2019 were reviewed retrospectively, and 61 patients who developed massive post-ES bleeding were enrolled. These patien...

ConclusionCity water was the source of biofilm formation of D acidovorans in RO machines that was not possible to eradicate. Application of incoming water filters was an effective preventive strategy. Replacement of RO machines after 4 failed disinfection attempts is the most cost-effective strategy as well for persistent positive water cultures. The HD remains RO water culture negative 2 years after changes.

In this study, polyetherimide/polyvinylpyrrolidone (PEI/PVP) hollow fiber membranes (HFMs) were synthesized by the phase inversion method. The synthesized PEI/PVP membranes were characterized by morphological analysis and transport properties (hydraulic permeability and solute rejection coefficient). Their performance in uremic solutes separation was also investigated. Urea removal and clearance were measured. Surface properties were analyzed by infrared spectroscopy (FT-IR), water uptake, zeta potential and protein adsorption. The results showed that PEI/PVP membranes had transport properties comparable to those of curren...

ConclusionsThis study reveals that TAFRO syndrome is more severe and has more systemic symptoms than other iMCD, most cases need active treatment, and their prognoses are poor. Lenalidomide based regimen may be as a promising new therapy for TAFRO syndrome.

CONCLUSIONS: In MHD patients, the circulating levels of ECB ligand, 2-AG, may play an important role in determining body mass and risk of mortality. These observations were unique to 2-AG as similar findings were not obtained with serum AEA. Future studies need to investigate the mechanisms responsible for these associations and examine the modulation of the ECB system as a potential target for therapy in ESRD. Abstract RATIONALE: End-stage renal disease (ESRD) patients have high annual mortality mainly due to cardiovascular causes. The acute effects of obstructive and central sleep apnea on cardiac function in ESRD patients have not been determined. We therefore tested, in patients with ESRD, the hypotheses that (1) sleep apnea induces deterioration in cardiac function overnight and (2) attenuation of sleep apnea severity by ultrafiltration (UF) attenuates this deterioration. METHODS: At baseline, ESRD patients, on conventional hemodialysis, with left ventricular ejection fraction (LVEF)>45% had polysomnography ...
