Telemedicine and Disparities in Visit Attendance at a Rural Pediatric Primary Care Clinic During the COVID-19 Pandemic
DISCUSSION: The adoption of telemedicine did not reduce pre-existing disparities in visit attendance. Further work is needed to identify the reasons for the disparities and improve visit attendance of historically disadvantaged patient populations.PMID:37464516 | DOI:10.1353/hpu.2023.0048

The Integrated Care for Kids Model: Addressing Fragmented Care for Pediatric Medicaid Enrollees in Seven Communities
J Health Care Poor Underserved. 2023;34(1):503-509. doi: 10.1353/hpu.2023.0034.ABSTRACTChildren with complex health and behavioral health conditions face challenges accessing coordinated health and community-based services. Adverse childhood experiences (ACEs), social determinants of health, and care delivery silos can lead to negative long-term outcomes. This paper provides an update on the Integrated Care for Kids (InCK) model from the Centers for Medicare & Medicaid Services' Innovation Center and discusses barriers and facilitators to program implementation in the seven communities. The three core InCK activiti...

State-Specific Medicaid/CHIP Eligibility Criteria and Their Impact on Health Care Access for Foreign-born Children Living in the United States
The objective of this study was to determine the effect of these policies on health care access.METHODS: Data from the National Survey of Children's Health (NSCH) were used to assess associations between health care access outcomes and three categories of state health insurance eligibility: restrictive (only U.S. citizens plus immigrants who "qualified" after five-year waiting period), semi-restrictive (same as restrictive except no waiting period), and inclusive (all children).RESULTS: When compared with restrictive states, foreign-born children in inclusive states were significantly more likely to have current insurance,...

The Effectiveness of Collaborative Care in Publicly Insured Populations
This study examines benchmark data from a behavioral health medical group embedded within primary care practices across the United States using a large dataset of patients treated in collaborative care. The study explores the effectiveness of collaborative care in reducing depression and anxiety symptoms by comparing Medicare, Medicaid, and privately insured populations, seeking to inform the importance of supporting advocacy for continued Medicaid fee adoption for collaborative care.PMID:37464532 | DOI:10.1353/hpu.2023.0042

The Use of Interpreters in Health Centers: A Mixed-Methods Analysis
J Health Care Poor Underserved. 2023;34(1):224-245. doi: 10.1353/hpu.2023.0015.ABSTRACTHealth centers serve millions of patients with limited English proficiency (LEP) through highly variable language services programs that reflect patient language preferences, the availability of bilingual staff, and very limited sources of third-party funding for interpreters. We conducted a mixed-methods study to understand interpreter services delivery in federally qualified health centers during 2009-2019. Using the Uniform Data System database, we conducted a quantitative analysis to determine characteristics of centers with and with...

Does Private Insurance Provide More Care?
This study examines whether its beneficiaries realize similar health care access as their counterparts with private insurance. We draw on the 2014-2017 Medical Expenditure Panel Survey and control for observable confounding factors across coverage types by estimating and applying entropy weights. Medicaid beneficiaries were moderately more likely to report a usual source of care (74.5% vs 68.1%, p<.01) and approximately equally as likely to report a checkup in the past year as the privately insured (43.5% vs 44.5%, p>.1), each significantly more likely than the uninsured. Medicaid beneficiaries had significantly ...

Telehealth Use and Access to Care for Underserved Populations Before and During the COVID-19 Pandemic
J Health Care Poor Underserved. 2023;34(1):132-145. doi: 10.1353/hpu.2023.0009.ABSTRACTWhile several studies have documented the rapid growth in telehealth visits during the pandemic, none have examined its relationship with greater overall access to care among vulnerable populations. We use Association of American Medical Colleges' Consumer Survey data to examine the relationship between access to care and telehealth use before and during the pandemic. The proportion of survey respondents who were always able to get medical care when needed was slightly lower in 2020 compared with prior years while telehealth use rose dra...
