Each year, 1·0–2·0% of individuals with atrial fibrillation and 0·1–0·2% of those with venous thromboembolism who are receiving one of the novel oral anticoagulants (dabigatran, rivaroxaban, or apixaban) can be expected to experience an acute ischemic stroke. The THRIVE score strongly predicts outcomes in patients treated with the Solitaire device in the SWIFT and STAR trials
ConclusionsThe THRIVE score strongly predicts clinical outcome and mortality in patients treated with the Solitaire device in the SWIFT and STAR trials. The lack of interaction between THRIVE and procedure‐specific elements such as vessel recanalization or device choice makes the THRIVE score a reasonable candidate for use as a patient selection criterion in stroke clinical trials.

ConclusionsThese results suggest that the larger proportion of patients arriving within 4·5 h in the centralized model might be related to a lower threshold to use emergency services to transport stroke patients and partly to preferential referral of thrombolysis candidates.

Do‐not‐resuscitate (DNR) orders in patients with intracerebral hemorrhage
ConclusionsIn addition to confirming the role of known intracerebral hemorrhage prognostic factors in do‐not‐resuscitate decision‐making, our results demonstrate that do‐not‐resuscitate orders led to less active care of intracerebral hemorrhage patients.

Predictors of in‐hospital death and symptomatic intracranial hemorrhage in patients with acute ischemic stroke treated with thrombolytic therapy: Paul Coverdell Acute Stroke Registry 2008–2012
ConclusionsIn this study of acute ischemic stroke patients, older age, male gender, National Institutes of Health Stroke Scale score, history of myocardial infarction or coronary artery disease, and history of atrial fibrillation were associated with increased in‐hospital death among patients receiving intravenous tissue plasminogen activator. Among patients treated with intravenous tissue plasminogen activator, in‐hospital mortality and symptomatic intracerebral hemorrhage rates were similar between those treated within three‐hours of time last known to be well and those treated between three and 4·5 hours after this time.

BackgroundIn adults, intraventricular thrombolytic therapy with recombinant tissue plasminogen activator (rtPA) facilitates resolution of intraventricular haemorrhage (IVH), reduces intracranial pressure, decreases duration of cerebrospinal fluid diversion, and may ameliorate direct neural injury.
