O09/90 Thrombectomy in ischemic stroke patients with large core but minor or absent ischemic changes on non-enhanced computed tomography
ConclusionIn patients with ASPECTS≥6, core volumes did not significantly modify the treatment effect of recanalization. Reperfusion and higher core volume were significantly associated with core overestimation which may explain the treatment effect of MT for patients with large ischemic core but minor ischemic changes on NECT.Abstract O09/90 Figure 1Abstract O09/90 Figure 2Disclosure of InterestNA.

P-012 stent retrievers in clinical practice: are results from recent trials reproducible in a community hospital referral network?
ConclusionsEndovascular treatment of LVO AIS based upon selection with CT perfusion imaging and modern stent retriever technology can be feasible, safe, and effective in the community setting. Times from imaging to groin puncture and reperfusion were longer compared to recent trials, however mRS at 90 days and sICH ratees were comparable. Good outcomes were noted despite long stroke onset to CTP times for many patients, emphasizing the potential role of CTP imaging in identifying patients with favorable perfusion and small core infarct who can benefit from IA therapy at extended time periods. These data suggest that a &lsq...

E-023 Predictors of outcomes in tandem anterior circulation occlusions following mechanical thrombectomy
In this study, we present outcomes of patients undergoing tandem mechanical thrombectomy. We demonstrate that stenting is associated with improved final reperfusion, but increased odds of asymptomatic ICH. Additional clinical trials should be performed to optimize the procedure and improve patient outcomes.Disclosures J. Catapano: None. D. Farhadi: None. A. Naik: None. S. Koester: None. E. O-027 Contrast density and volume on post-thrombectomy dual-energy head computed tomography predict delayed hemorrhagic transformation
ConclusionMaximum contrast density predicted the occurrence of delayed petechial hemorrhage, while contrast volume predicted the occurrence and volume of delayed parenchymal hematoma. The volume of contrast on DEHCT can serve as a useful predictor of DHT and may have implications for patient management.Abstract O-027 Figure 1Disclosures S. Ahn: None. S. Roth: None. Y. Ko: None. A. O-001 Impact of Recanalization in Patients with Pretreatment DWI-ASPECTS
ConclusionPatients with a pretreatment DWI-ASPECTS ≤6 may still benefit of EVT when a good recanalization is achieved. In particular, EVT-induced recanalization was associated with a reduced rate of mortality without increased risk of symptomatic intracerebral hemorrhage.DisclosuresJ. Desilles: None. A. P-013 Use of the Solitaire Device for Emergency Revascularization of the Superior Mesenteric Artery
ConclusionsThe techniques and equipment used for acute stroke intervention may be suitable for acute recanalization of peripheral vessels, under the right circumstances. As with many surgical interventions, a multidisciplinary approach may at times yield a novel and effective strategy for a difficult clinical problem.DisclosuresJ. Dalfino: None. A. Paul: None. J. O-037 Comparative safety and efficacy of combined intravenous thrombolysis and mechanical thrombectomy with mechanical thrombectomy alone in emergent large vessel occlusion
Conclusions Our multicenter study appears to provide observational evidence that IVT pretreatment may improve MT outcomes in ELVO patients. The comparative efficacy of MT+IVT vs. MT monotherapy can be established only in the settings of a randomized-controlled clinical trial. Disclosures N. Goyal: None. G. Tsivgoulis: None. D. O-030 Super-selective middle meningeal artery embolization for chronic subdural hematoma using n-BCA with the sugar push technique: single center experience of 61 consecutive patients
ConclusionMMA embolization using diluted n-BCA with concomitant Dextrose 5% injection (Sugar-Rush) is associated with a high degree of distal penetration and complete branch occlusion and minimal risk of cranial nerve palsy or other thrombo-embolic complications. Clinical trials are warranted to compare long-term efficacy of this procedure with standard of care management.Disclosures S. Majidi: None. S. Matsoukas: None. R. De Leacy: None. P. O-064 AI based gaze deviation detection to aid LVO diagnosis in NCCT
ConclusionsIpsiversive Gaze deviation on NCCT is a good predictor of LVO due to proximal vessel occlusions in ICA terminus and M1 occlusions. However, it is a poor predictor of LVO due to M2 occlusion. We report an AI algorithm that can identify this clinical sign on NCCT. These findings can aid in the triage of LVO patients and expedite the identification of EVT candidates.Abstract O-064 Figure 1Disclosures U. Upadhyay: None. S. Golla: None. S. Kumar: None. K. E-108 Ruptured intracranial aneurysm presenting as isolated acute subdural hemorrhage
ConclusionIn rare cases, ruptured intracranial aneurysms can be associated with isolated subdural hemorrhage. Common treatment options include endovascular coiling and microsurgical clipping. However, endovascular repair is often preferred especially when the patient may not be able to tolerate a surgical procedure, as was the case with this patient. In this case, the patient presented with stroke-like symptoms and was found to have a subdural hemorrhage. After emergent craniotomy to evacuate the hematoma, successful endovascular coiling was performed, and the patient was stabilized for further management>Disclosures D....

P65 "Direct Transfer to Angio Suite" (DTAS) enabling via utilization of Philips advanced 3D neuro-imaging prototype technology
Conclusions3D neuro-imaging prototype technology has provided an almost multi-detector CT level of anatomical structures spatial and contrast resolution, during pre-procedural ASPECT assessment, clot localization, collaterals and post-procedural hemorrhagic components evaluation during DTAS for mechanical thrombectomy, significantly reducing the door-to-puncture timing. This new technology significantly improves and maximizes the triaging of these LVO patients in the DTAS pathway.ReferencesPatrick Nicholson, Nicole M Cancelliere, John Bracken, et al. E-164 Symptomatic developmental venous anomaly thrombosis associated with covid-19 infection
We describe a milder case of DVA thrombosis in the setting of COVID-19 infection presenting with unilateral hemiparesis requiring medical management alone. A 41-year-old female with hypertension who presented with left hemiparesis. Non-contrast computed tomography (CT) of head revealed linear calcifications in the right centrum semiovale consistent with DVA. Magnetic resonance imaging (MRI) revealed an acute ischemic stroke in the right corona radiata adjacent to the vascular abnormality. Extensive workup for other causes of ischemic stroke was unrevealing. Cerebral angiography ruled out arteriovenous shunting and did not ...

P124/272 Initial encounter with a cutting-edge mobile head CT scanner (Somatom On.site) in a neuro-intensive care unit
ConclusionThe study demonstrates the clinical effectiveness of modern mobile CT scanners in neurointensive care units. The Somatom On.site provides sufficient image quality, saves time and supports timely decision making. The integration of mobile CT scanners in NICUs improves patient care and workflow. Prospective studies are needed to validate these findings and assess the long-term impact in NICUs and other units, such as stroke units.Disclosure of InterestCK serves as consultants for Acandis GmbH (Pforzheim, Germany) and proctor for MicroVention Inc./Sequent Medical (Aliso Viejo, CA, USA).
