Rethinking the role of CT perfusion in the management of emergent large vessel ischemic stroke
The treatment of emergent large vessel occlusion (ELVO) was revolutionized by endovascular thrombectomy (EVT). The identification of eligible patients for EVT has sparked significant interest in determining the neuroimaging modality that offers optimal results. The American Stroke Association recommend the use of both non-contrast CT and CT angiogram for patients coming in the early window (0–6 hours).1 In the late window (6–24 hours) the randomized trials, DAWN and DEFUSE 3, showed clinical benefit utilized some form of quantitative estimation of the core infarct.2 3 Consequently the society recommends automat...

Stroke imaging modality for endovascular therapy in the extended window: systematic review and meta-analysis
Conclusions Our study suggests that, in anterior circulation LVO, the rates of functional independence may be similar when patients are selected using ADVI or NCCT for EVT in the extended time window. A simplified triage protocol does not seem to increase mortality or sICH. Protocol registration number (PROSPERO ID: CRD42021236092).

P-039 Flow diverters for treatment of ICA loop dissections or ICA dissections with associated pseudoaneurysm
ConclusionUse of flow diverters, in some cases in conjunction with (carotid) stents, is an effective treatment option for cervical carotid artery dissections, especially loop dissections with or without associated pseudo aneurysms.Disclosures J. P-028 A Comparison of Outcomes between Stent Types in the Treatment of Extracranial Vertebral Artery Stenosis: A Systematic Review
Conclusion We found that ECVA stent placement has low rates of stroke or death for all stent groups. There were non-significant trends towards lower any stroke-or-death rates in follow up and lower rates of restenosis in the newer generation stents. DES had lower vertebrobasilar strokes in follow up compared to newer generation NDES (p = 0.03) but not older generation NDES. Disclosures J. Kim: None. M. Memon: None. H. Zacharatos: None. K. E-073 Clot bank collaborative registry protocol: Novel method for evaluating ischemic thrombus
ConclusionWe have developed a novel tissue banking protocol to preserve microbiota from arterial thrombi retrieved from ischemic stroke patients. While there is no consensus which class of pathogens is implicated in the susceptibility to stroke, multiple studies including our own, have shown bacterial fragments and communities in cerebral thrombi. The specimens captured during standard-of-care thrombectomy allow us to characterize and functionally define the microbiota associated with cerebral thrombi in ischemic stroke patients.Disclosures M. E-124 Timing surgery and hemorrhagic complications in endocarditis with concomitant cerebral complications
ConclusionsPatients with radiographic evidence of ischemic stroke from septic emboli can safely undergo valvular surgery for IE without increased risk of symptomatic hemorrhage. We advocate for baseline CTA screening to evaluate for IIA in patients who present with a primary diagnosis of IE and propose a management algorithm.Disclosures B. Lucke-Wold: None.

E-138 Intracerebral hemorrhage in patients transferred for mechanical thrombectomy
ConclusionsIn a large series of stroke patients being specifically transferred for mechanical thrombectomy, the rate of ICH on arrival to the thrombectomy hospital was 1.2%. In less than 50% of those cases the decision to forgo intervention was related to the ICH finding. The results of our study adds to the concerns that repeat imaging delays treatment by demonstrating that the incidence of ICH is low, which may warrant reconsideration of current stroke workflows for large vessel occlusion transfers.Disclosures Y. P-026 pufs primary endpoint complications by aneurysm size
ConclusionsAll 6 major neurologic morbidity and neurologic mortality events in the PUFS trial was observed in 84 patients harboring large aneurysms, for 7% complication rate in this subgroup. Patients with giant aneurysms suffered no such complications, however, their parent vessel occlusion rates were higher at 18.2% (4/22). More aggressive or more prolonged antiplatelet treatment may be necessary in patients with giant aneurysms treated with flow diversion alone.DisclosuresT. Becske: 2; C; Proctor for Covidien. M.
