Obituary Gordon Ostlere, alias Richard Gordon
Anaesthetist and author of the successful series of Doctor books. Born in London on Sept 15, 1921, he died after a stroke in Kent, UK, on Aug 11, 2017, aged 95 years.

Comment Do we need to know whether nitrous oxide harms patients?
In The Lancet, Paul Myles and colleagues investigate the association between nitrous oxide exposure and cardiovascular complications such as non-fatal myocardial infarction, stroke, pulmonary embolism, cardiac arrest, and death, within 30 days of surgery, in patients with known or suspected coronary artery disease having major non-cardiac surgery under general anaesthesia. The rationale for this large, multicentre study, which involved more than 7000 patients from 45 centres, was the observation that short-term exposure to nitrous oxide led to significant increases in plasma homocysteine.

Perspectives The singular science of John Snow
John Snow was a scientist of rare dedication, who published more than 100 contributions to the medical literature in a career entirely supported by clinical earnings and terminated by a lethal stroke when he was just 45 years old. The range of clinical and scientific topics covered by his research was wide, although he is most remembered for developing the scientific basis of anaesthesia practice and working out the epidemiology and preventability of cholera. Snow's science was singular in two senses: it was singularly creative and productive; and it reflected a singular, unified scientific approach to the problems he addressed.
