Balloon ‐Expandable Transcatheter Aortic Valves Can Be Successfully and Safely Implanted Transfemorally Without Balloon Valvuloplasty
ConclusionsTAVI can be effectively and safely performed without a BAV and this results in reduced procedural and fluoroscopic times, although embolization to the brain is not reduced. There is a trend toward reduced stroke risk. (J Interven Cardiol 2016;29:319–324)

Improvements of Procedural Results With a New ‐Generation Self‐Expanding Transfemoral Aortic Valve Prosthesis in Comparison to the Old‐Generation Device
ConclusionsTAVR with the new Evolut R resulted in significantly lower paravalvular regurgitation. This may be due to redesigned cell geometry and higher precision during implantations, as well as the ability to resheath a malpositioned valve.

Outcomes in Elderly Patients With Severely Calcified Coronary Lesions Undergoing Orbital Atherectomy
ConclusionsThe elderly represented a sizeable number of patients who underwent orbital atherectomy. It is a safe and effective treatment strategy for elderly patients with severe CAC as the clinical outcomes were similar to their younger counterparts. A randomized trial should further clarify the role of orbital atherectomy in these patients.

Transcatheter aortic valve implantation in the United States: Predictors of early hospital discharge
CONCLUSIONSFemales, bleeding, blood transfusions, stroke, permanent pacemakers, mechanical circulatory support devices, renal failure were associated with lower odds for early discharge. Transfemoral approach and prior aortic valvuloplasty increased the likelihood for early discharge. Post‐TAVI length of stay was associated with significantly higher hospitalization costs.

Left atrial anatomy and patient ‐related factors associated with adverse outcomes with the watchman device—a real world experience
ConclusionCertain LAA morphologies and patient characteristics increase the risk for a MACCE following left atrial appendage occlusion (LAAO) with the Watchman™ device. Anterior chicken wing morphology of the LAA and low BMI <30 kg/m2 were independent predictors of MACCE in the multivariate regression model.

ConclusionsCIN is not an uncommon complication associated with PVI, can be reliably predicted from pre‐procedural variables, including a contrast dose of greater than three times the CCC and is strongly associated with the risk of in‐hospital death, MI, stroke, transfusion, and increased hospital length of stay.

Impact of right and left ventricular systolic dysfunction on perioperative outcome and long ‐term survival after transcatheter aortic valve replacement
ConclusionsPeri‐operative mortality and risk of stroke after TAVR are not adversely affected by preexisting RV or LV dysfunction. Long‐term survival is impaired in patients with RVSD. RVSD but not LVSD is an independent risk factor for late mortality. TAVR should be the preferred therapy for patients with RVSD and LVSD, especially when patient is suitable for TF.

A retrospective analysis of Impella use in all ‐comers: 1‐year outcomes
ConclusionFor patients with need for an Impella device, regardless of the indication, early implantation is associated with better in hospital and 1 year outcomes as compared to when the device is implanted late as a bailout.

Head ‐to‐head comparison of prasugrel versus ticagrelor in patients undergoing percutaneous coronary intervention: A meta‐analysis of randomized controlled trials
ConclusionsAlthough the efficacy was similar between prasugrel and ticagrelor, prasugrel may be associated with a lower risk of bleeding compared with ticagrelor during short‐ to mid‐term follow‐up period after PCI. Further studies are warranted in a larger patient population during longer‐term follow up to validate these findings.

Bivalirudin versus heparin in patients undergoing percutaneous transcatheter aortic valve interventions: A systematic review and meta ‐analysis
ConclusionsIn patient undergoing aortic valve interventions, no difference was seen between the use of bivalirudin and heparin as the procedural anticoagulant agent, except for a significant lower myocardial infarction events when bivalirudin was used. Further large randomized trials are needed to confirm current results.

Rotational Atherectomy for Left Main Coronary Artery Disease in Octogenarians: Transradial Approach in a Tertiary Center and Literature Review

The Current Use of Impella 2.5 in Acute Myocardial Infarction Complicated by Cardiogenic Shock: Results from the USpella Registry
ConclusionsThe results of our study suggest that early initiation of hemodynamic support prior to PCI with Impella 2.5 is associated with more complete revascularization and improved survival in the setting of refractory CS complicating an AMI.
