Effect of prior clopidogrel use on outcomes in medically managed acute coronary syndrome patients
Conclusions Patients receiving clopidogrel before admission for ACS and subsequently treated only medically are at higher risk for CV events versus those not previously receiving clopidogrel. More potent antiplatelet inhibition with prasugrel versus clopidogrel did not significantly reduce this risk. Trial registration number NCT00699998.

Left atrial appendage occlusion in high-risk patients with non-valvular atrial fibrillation
Conclusions Our data suggest LAA occlusion in high-risk patients with NVAF not suitable for OACs is feasible and associated with low complication rates as well as low rates of stroke and major bleeding at long-term follow-up.

A tragedy of modern cardiology: using ejection fraction to gauge left ventricular function in mitral regurgitation
Mechanics The normal mitral valve provides unidirectional flow from left atrium (LA) to left ventricle (LV) in a smooth and efficient fashion. Mitral incompetence allows backward flow into the LA, requiring the LV to pump additional volume to compensate for that lost to regurgitation (MR). Conclusions After incident CHD, women have lower risk of composite MACE and recurrent CHD outcomes but higher risk of stroke, heart failure, and all-cause mortality compared with men.

Trends in the pharmacological management of atrial fibrillation in UK general practice 2008-2018
Conclusions There has been an increase in the proportion of patients with AF appropriately prescribed anticoagulants following National Institute for Health and Care Excellence and European Society of Cardiology guidelines, which correlates with improvements in mortality and stroke outcomes. Beta-blockers appear increasingly favoured over digoxin for rate control. There has been a steady decline in GP prescribing rates for rhythm control drugs, possibly related to concerns over efficacy and safety and increased availability of AF ablation.

Sex-specific differences in adverse outcome events among patients with atrial fibrillation
Conclusion In this large study of patients with established AF, women had a lower risk of death than men, but there were no sex-specific differences in other adverse outcomes.

Population-wide cohort study of statin use for the secondary cardiovascular disease prevention in Scotland in 2009-2017
Conclusions Statin use remains suboptimal for the secondary ASCVD prevention, particularly in women and older patients, and following ischaemic stroke and PAD hospitalisations. Improving this would offer substantial benefits to population health at low cost.

The unnatural history of an atrial septal defect: Longitudinal 35 year follow up after surgical closure at young age
Conclusions Very long-term outcome after surgical ASD closure in childhood shows good survival and low morbidity. Early surgical closure prevents pulmonary hypertension and reduces the occurrence of supraventricular arrhythmias. Early postoperative arrhythmias are predictive for the need for pacemaker implantation during early follow-up, but the rate of late pacemaker implantation remains low. Although RVEF was unexpectedly found to be decreased in one-third of patients, the functional status remains excellent.

Patent foramen ovale closure and brain ischaemic lesions
To the Editor The recently published meta-analysis by Wolfrum et al assessed the ability of percutaneous closure of patent foramen ovale (PFO) to reduce the incidence of recurrent stroke based on three randomised controlled clinical trials (RCT). This analysis did not demonstrate a superiority of the closure of PFO compared with medical therapy after 2–4 years of follow-up in patients with cryptogenic embolism.1 The closure was associated with lower, albeit nonsignificant, incidence of nonfatal stroke. Meta-analysis of secure randomised controlled trials of {beta}-blockade to prevent perioperative death in non-cardiac surgery
Conclusions Guideline bodies should retract their recommendations based on fictitious data without further delay. This should not be blocked by dispute over allocation of blame. The well-conducted trials indicate a statistically significant 27% increase in mortality from the initiation of perioperative β-blockade that guidelines currently recommend. Any remaining enthusiasts might best channel their energy into a further randomised trial which should be designed carefully and conducted honestly.

Atrial fibrillation and thromboembolism in patients with hypertrophic cardiomyopathy: systematic review
Conclusions AF is common in HCM and associated with high thromboembolic risk. LA dimension and age are independently associated with AF but the literature is insufficient to create robust clinical tools to predict AF or thromboembolism. Most data suggest that AF patients should be anticoagulated.
