Exercise oxygen pulse kinetics in patients with hypertrophic cardiomyopathy
Conclusion
None of the frequently used CPET parameters, either as absolute values or dynamic relationships, were associated with LVOTO. Differently, an abnormal temporal behaviour of O2-pulse during exercise, which is strongly related to inadequate SV increase, correlates with reduced functional capacity (peak and anaerobic threshold VO2 and workload) and increased VE/VCO2 slope, identifying more advanced disease irrespectively of LVOTO.

Contemporary demographics, diagnostics and outcomes in non-bacterial thrombotic endocarditis
Conclusions
NBTE was more prevalent in females and embolic complications were the most frequent clinical finding. Overall, patients with NBTE had a poor prognosis, particularly in those with lung cancer or metastatic tumours. Further studies in patients with NBTE are needed given its morbidity and mortality.

Proteomics profiling reveals a distinct high-risk molecular subtype of hypertrophic cardiomyopathy
Conclusions
Our prospective plasma proteomics study not only exhibited the presence of HCM molecular subtypes but also identified pathobiological mechanisms associated with a distinct high-risk subtype of HCM.

Transcatheter versus surgical closure of atrial septal defect in adult patients with tricuspid regurgitation
Conclusions
While TAP was effective for the treatment of significant TR, transcatheter ASD closure also significantly reduced TR as well. Transcatheter ASD closure may be considered an alternative treatment option in patients with moderate or severe TR.

Heartbeat: hospitalisation for COVID-19 is associated with an increased risk of subsequent adverse cardiovascular events
In order to assess whether previous COVID-19 infection might be associated with a higher risk of subsequent adverse cardiovascular disease (CVD) events, Raisi-Estabragh and colleagues1 analysed incident CVD events in 17 871 UK Biobank cases with prospective follow-up from March 2020 to March 2021. Hospitalisation for COVID-19 was associated with an increased risk of several adverse CVD events, including myocardial infarction, stroke, heart failure, atrial fibrillation, venous thromboembolism and mortality (figure 1). Risk was highest in the first 30 days after infection, but remained higher than matched controls over an av...

Not to be sneezed at: cardiovascular disease after COVID-19 infection
COVID-19 has changed our way of life since it was first identified in December of 2019. While our understanding of the manifestations and outcomes of the immediate acute illness has improved, we are still learning about the medium to long-term impact of this diagnosis on patients’ health. For some time, it has been suggested that COVID-19 may be associated with incident cardiovascular events such as venous thromboembolism, stroke and myocardial infarction.1 However, the absolute risk of these events and whether an excess risk is present are challenging to determine without a contemporary reference population. This is...

Prediction of incident atrial fibrillation with cardiac biomarkers and left atrial volumes
Conclusions
In the general population, echocardiographic left atrial volumetric indices and NT-proBNP, but not cTnT and GDF-15, associate with prevalent atrial fibrillation and with risk of incident atrial fibrillation. cTnT and NT-proBNP provide incremental prognostic information to echocardiography.

Cryoballoon Pulmonary Vein Isolation as First-Line Treatment for Typical Atrial Flutter
Conclusion
Cryoballoon PVI as first-line treatment for AFL is equally effective compared with standard CTI ablation for preventing recurrence of atrial arrhythmia and better at preventing new-onset AF.
Trial registration number
NCT03401099.

Emergent readmission and long-term mortality risk after incident atrial fibrillation hospitalisation
Conclusion
This study highlights the large burden of unplanned all-cause and cardiovascular-specific readmissions within 2 years after being hospitalised for incident AF and their associated adverse impact on mortality. Concomitant comorbidities are independently associated with unplanned hospitalisations and mortality, which supports integrated multidisciplinary management of comorbidities, along with AF-targeted treatments, to improve long-term outcomes in patients with AF.

Lower birth weight is linked to poorer cardiovascular health in middle-aged population-based adults
Conclusions
Lower birth weight was associated with greater risk of incident MI and unhealthy LV phenotypes; effects were partially mediated through cardiometabolic disease and systemic inflammation. These findings support consideration of birth weight in risk prediction and highlight actionable areas for disease prevention.

Prevalence, outcomes and costs of a contemporary, multinational population with heart failure
Conclusions
We estimate that 1–2% of the contemporary adult population has HF. These individuals are at significant risk of adverse outcomes and associated costs, predominantly driven by hospitalisations for HF or CKD. There is considerable public health potential in understanding the contemporary burden of HF and the importance of optimising its management.

Proof of concept study on coronary microvascular function in low flow low gradient aortic stenosis
Conclusions
CMD is more severe in patients with LFLGAS compared with NFHGAS and is associated with low-flow state, left atrial dysfunction and reduced cardiac performance.

Heartbeat: prediction of atrial fibrillation risk
One of the challenges in prevention of stroke is identification of patients at risk of atrial fibrillation (AF) shortly before the onset of the arrhythmia so that prevention therapies can be promptly instituted. In this issue of Heart, Nadarajah and colleagues1 report the development and validation of an algorithm based on data from the UK Clinical Practice Research Datalink GOLD dataset that included 2 081 139 individuals of whom 7386 developed AF within 6 months. This new algorithm-- Future Innovations in Novel Detection of Atrial Fibrillation (FIND-AF) – identified a high-risk cohort with a 20-fold higher 6 month ...

Patient with altered mental status
Clinical introduction A woman in her 60s with history of diabetes mellitus, hypertension and hypothyroidism presented to the emergency department after being found confused, non-verbal and with low urine output in a nursing home. Her last known well was around 6 o’clock in the morning before presentation. Her temperature was 94.5°F, blood pressure (BP) was 179/80 mm Hg, heart rate (HR) was 50/min, and saturation was 97% on room air. She was cold to touch. The lungs were clear to auscultation bilaterally. She was noted to have gaze preference initially to the right, then to the left, right-sided weakness and left ...
