Evaluation of antithrombotic use and COVID-19 outcomes in a nationwide atrial fibrillation cohort
Conclusions Pre-existing AT use may be associated with lower odds of COVID-19 death and, while not evidence of causality, provides further incentive to improve AT coverage for eligible individuals with AF.

Association of carbohydrate and saturated fat intake with cardiovascular disease and mortality in Australian women
Conclusions In middle-aged Australian women, moderate carbohydrate intake (41.0%–44.3% of TEI) was associated with the lowest risk of CVD, without an effect on total mortality. Increasing saturated fat intake was not associated with CVD or mortality and instead correlated with lower rates of diabetes, hypertension and obesity.

Sex and gender matter in cardiovascular disease and beyond
Sex and gender are fundamental drivers of virtually all major causes of death and disease, while gender equality has been shown to improve the health of both women and men at the population level. The term ‘sex’ is generally used to describe biological characteristics, while ‘gender’ is used to address social constructs. Sex and gender are intertwined and interconnect with other key drivers of health, such as age, socioeconomic position, race and ethnicity. Over the past decade, many clinically meaningful sex differences in several aspects of cardiovascular disease (CVD) have been uncovered. Althoug...

Hypertensive response to exercise in adult patients with repaired aortic coarctation
Conclusions A hypertensive response to exercise was present in nearly half of the patients in this large, prospective cohort of adults with repaired CoA. Risk factors for increased peak exercise SBP were male sex, absence of a bicuspid aortic valve and elevated resting SBP. Increased peak exercise SBP independently predicted hypertension at follow-up. These results support close follow-up of patients with a hypertensive response to exercise to ensure timely diagnosis and treatment of future hypertension.

Impact of cardiometabolic multimorbidity and ethnicity on cardiovascular/renal complications in patients with COVID-19
Conclusions In hospitalised patients with COVID-19, cardiovascular complications or death impacts just under half of all patients, with the highest risk in those of South Asian or Black ethnicity and in patients with cardiometabolic multimorbidity.

Atrial fibrillation following transcatheter atrial septal defect closure: a systematic review and meta-analysis
Conclusions The incidence of new-onset AFi/AFl is relatively low following transcatheter closure of secundum ASDs. The rate of de novo AFi/AFl, however, was significantly higher in elderly patients. Shorter follow-up time was associated with a higher reported incidence of AFi/AFl.

BMJ Rapid Recommendations on use of proprotein convertase subtilisin/kexin 9 inhibitors and ezetimibe to reduce cardiovascular risk
The new BMJ Rapid Recommendations addressed the following question: should we add another lipid-lowering drug in adults with low-density lipoprotein-cholesterol (LDL-C) over 1.8 mmol/L using a maximal dose of statins or intolerant to statins?1 Why is this question important? Both proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors and ezetimibe have been shown to reduce LDL-C by approximately 60% and approximately 20%, respectively, and correspondingly may reduce major adverse cardiovascular events (MACE), including all-cause death, cardiovascular death, myocardial infarction (MI) and stroke. However, the bene...

Air pollution and cardiovascular disease: the Paul Wood Lecture, British Cardiovascular Society 2021
Air pollution is associated with up to 8.8 million excess deaths worldwide each year and is a major contributor to the global burden of disease. Cardiovascular conditions are the predominant cause for air pollution-related deaths and there is an urgent need to address the silent pandemic of air pollution on cardiovascular health. Air pollution exposure is associated with acute events like acute coronary syndrome and stroke, and with chronic conditions, such as atherosclerosis and heart failure. Several potential mechanisms have been proposed that link particle inhalation to cardiovascular disease including oxidative stress...

A strange way of breathing in a patient with advanced systolic heart failure
Clinical introduction A man in his 70s with a left bundle branch block and advanced valvular systolic heart failure (HF) presented with a subacute HF and low cardiac output (CO) (peripheral hypoperfusion, high lactic levels, symptomatic hypotension). Echocardiography confirmed severe reduction in left ventricular ejection fraction, global longitudinal strain and stroke volume, as assessed by outflow tract pulse Doppler. Acute HF therapy with dobutamine and levosimendan was started, leading to progressive haemodynamic improvement. After suspension of inotropes, under conditions of haemodynamic stability, a cardiopulmonary r...

Heartbeat: cardiovascular maternal health and disparities in clinical outcomes
Cardiovascular disease continues to account for a high proportion of pregnancy-related deaths in women in the USA. In addition to pre-existing heart disease, gestational hypertension and diabetes, as well as pre-eclampsia and eclampsia, are associated with an increased risk of adverse cardiovascular outcomes both in the short and long-term. In this issue of Heart, Marschner and colleagues1 report an incidence of cardiometabolic conditions (hypertensive disease and diabetes) of 224.3 (95% CI 221.3 to 227.3) per 1000 births among 74 510 women, mean age 26.4 years (SD 5.5) in a cross-sectional study of Medicare patients from ...

Pregnancy-related cardiovascular conditions and outcomes in a United States Medicaid population
Conclusion This US cohort of Medicaid-funded women have a high incidence of severe cardiovascular conditions during pregnancy. Cardiometabolic conditions of pregnancy conferred threefold higher odds of severe cardiovascular outcomes.

Clinical outcomes in spontaneous coronary artery dissection
Conclusions In this large prospective cohort of patients with SCAD, prognosis was overall favourable, with events mainly driven by reinfarctions or unplanned revascularisations. History of hypothyroidism, proximal vessel involvement, type 2 IMH and DAPT at discharge were associated with MACCE. Trial registration number NCT03607981.

Inter-racial differences in patients undergoing transcatheter aortic valve implantation
Conclusions There were significant differences in baseline and procedural factors among Asian and non-Asian patients who underwent TAVI. Observed inter-racial differences in clinical outcomes were largely explained by baseline differences in clinical, anatomical and procedural factors. Trial registration number NCT03826264 (https://wwwclinicaltrialsgov).

Heartbeat: knowledge gaps for non-bacterial thrombotic endocarditis
Non-bacterial thrombotic endocarditis (NBTE) contributes to adverse outcomes in patients with systemic inflammatory conditions, antiphospholipid syndrome or advanced malignancy (figure 1). Diagnosis is challenging and there is little data to guide prevention of adverse events, particularly stroke. In this issue of Heart, Quintero-Martinez and colleagues1 report the clinical characteristics and outcomes in 48 adults with NBTE identified at the Mayo Clinic over a 7 year period. Most patients (75%) were women with a mean age of 60 years and most cases were associated with malignancy (52%) or a systemic inflammatory disease (3...

Effects of salt substitutes on clinical outcomes: a systematic review and meta-analysis
Conclusions The beneficial effects of salt substitutes on blood pressure across geographies and populations were consistent. Blood pressure-mediated protective effects on clinical outcomes are likely to be generalisable across population subgroups and to countries worldwide. Trial registration number CRD42020161077.
