Stress-induced cardiomyopathy accompanied by heat stroke
We describe a case with high body temperature and coma, who also presented with wide QRS tachycardia and shock. He was initially treated for heat stroke and wide QRS tachycardia. Serial electrocardiographic change and echocardiographic findings suggested this was complicated with stress-induced cardiomyopathy. This case demonstrates the importance of recognizing that serious myocardial damage can result from heat stroke. <Learning objective: Heat stroke can cause multiple organ failure and the presentation of circulatory failure in heat stroke may be the sign of myocardial dysfunction. Acute ischemic stroke what is hidden behind?
We present a case of a 58-year-old male patient that presented to ED with sudden onset of headache and left-sided hemiparesis, computed tomography (CT) demonstrated an ischemic stroke of the right middle cerebral artery. When the question of whether to start r-TPA or mechanical thrombectomy was discussed, a cardiac point-of-care ultrasound was performed in ED and showed a type A aortic dissection; immediately a CT aortic angiogram was performed and confirmed the diagnosis. Rivaroxaban for treatment of intraventricular thrombus in Chagas disease
We present a case of a 61-year-old man admitted for stroke 5 months after a renal embolism. An intraventricular thrombus was observed, probably the source of the cerebral and renal embolisms. The patient refused warfarin and rivaroxaban was used instead. After 40 days of treatment the thrombus had dissolved, after 20 months of regular use of rivaroxaban no more embolic events were observed. Silent cardiac tumor with neurological manifestations
This article describes the case of a 61-year-old woman whose clinical presentation of an atrial myxoma was a stroke. Echocardiographic findings were highly suggestive of a cardiac myxoma. However, cardiac magnetic resonance showed unusual features for myxoma, since the mass was hyperintense in T1-weighted images and hypointense in T2-weighted sequences. Histology confirmed myxoma and the patient was surgically treated. Transient occlusion of the right coronary artery by a calcific bicuspid aortic valve mass – An interesting case of inferior STEMI
We report the case of a myocardial infarction caused by transient occlusion of the right coronary artery, secondary to a mobile calcified lesion attached to a bicuspid aortic valve. <Learning objective: The purpose of this article is to demonstrate the importance of echocardiography in patients presenting with an acute myocardial infarction, without significant coronary artery disease. Urgent echocardiography in this case ensured identification of a calcified bicuspid valve and mobile lesion that had transiently occluded the right coronary artery.
