Stroke vision, aphasia, neglect (VAN) assessment--a novel emergent large vessel occlusion screening tool: pilot study and comparison with current clinical severity indices
Conclusions The VAN screening tool accurately identified ELVO patients and outperformed a NIHSS ≥6 severity threshold and may best allow clinical teams to expedite care and mobilize resources for ELVO patients. A larger study to both validate this screening tool and compare with others is warranted.

Streamlining door to recanalization processes in endovascular stroke therapy
Conclusions This pilot study demonstrates that D2P times can be significantly reduced with a standardized multidisciplinary approach. There was no significant difference in the rate of 3-month good outcome, which is most likely due to the small sample size and confounding baseline patient characteristics.

O-020 Novel cone beam CT technology improves image quality for stroke assessment: a prospective series
ConclusionsThe latest generation of CB-CT scans & technology allow for exclusion of haemorrhages, stroke core definition and demonstration of brain perfusion and collaterals. These improvements suggest that CB-CT is acceptable for emergency stroke imaging assessment before mechanical thrombectomy, which may reduce door-to-groin puncture times and improve patient outcomes.ReferenceBourcier R, Goyal M, Liebeskind DS, et al. E-054 Correlation between Thrombus Density and Recanalization or Stroke Etiology in Acute Ischemic Stroke
ConclusionIn conclusion, our study found no relationship between thrombus attenuation and recanalization success or stroke etiology. While prior studies have suggested that higher attenuation is associated with good recanalization, our study could not find such a link.4 More studies are required to identify factors that predict successful recanalization in acute ischemic stroke patients.DisclosuresM. P34 Artificial intelligence in stroke networking: early experience in building networks of care
ConclusionsWhile Viz. ai is intended to identify and triage thrombectomy patients, the messaging app provides an opportunity to collaborate between institutions; care recommendations without transfer were provided in just as many cases as thrombectomy. In addition, the majority of stroke cases did not require transfer. Do you have any conflict of interest to declare?: No

P-019 Clinical significance of contrast staining after mechanical thrombectomy for acute ischemic stroke
ConclusionContrast staining is associated with the development of ICH, particularly in the early period after MT and worse clinical outcomes. While patients with ICH had worse functional outcomes at discharge, contrast staining itself was not associated with worse outcomes. Lasty, larger diameter clot retriever devices and catheters were not significantly correlated with the presence of contrast staining.Disclosures T. Lazaro: None. P. Cotton: None. C. English: None. K. E-176 Endovascular stent retriever thrombectomy of medium and small vessel occlusion of the middle cerebral artery with acute ischemic stroke: a case series
ConclusionsOur case series demonstrated that SRMT in SVO with disabling NIHSS is feasible and safe if performed carefully. All patient achieved TICI 3 perfusion and first pass effect was better observed in trap technique and use of long solitaire STD. Good function outcomes were achieved in all cases irrespective of the types of SRD used and passes required to achieve TICI 3 perfusion. Further studies are required.Disclosures Y. Lodi: None.

O-006 Analysis of Screened Patients from the Penumbra THERAPY Trial: Correlations of Clot Length Assessed by Thin-Section CT in a Sequential Series of Acute Stroke Patients
Conclusion This analysis further supports the idea of measuring hyperdense thrombus by use of thin-section NCCT. Occlusion site is a key predictor of clot length, which may explain the relative ineffectiveness of IV rtPA in more proximal anterior circulation large vessels. These findings suggest a sizeable population who may benefit from a bridging approach. Disclosures D. Frei: 3; C; Penumbra Inc. 4; C; Penumbra Inc. D. Heck: None. A. Carotid artery stenting in nonagenarians: are there benefits in surgically treating this high risk population?
Conclusions CAS in nonagenarian patients carried increased risk of perioperative ischemic events, compared with contemporary trial results in symptomatic and asymptomatic patients. Further, mean survival time postprocedure fell short of guidelines for receiving procedural benefit. Although larger scale multicenter research is needed, we recommend careful consideration of overall health status when contemplating stenting in nonagenarians.
