Health Coaching for Secondary Prevention in TIA Clinic (P5.148)
Conclusions: Structured health coaching for improving medication adherence of secondary stroke prevention medications can be a feasible in a community based outpatient setting for high cardiovascular risk patients. We will be implementing this care pathway in 2015 at Virginia Mason in Seattle, WA to evaluate the feasibility of the intervention, and assess adoption rate by patients and providers. Cognitive Deficits of Large-Vessel and Small-Vessel Vascular Dementia (P6.238)
CONCLUSIONS: Patients with small-vessel VaD are more impaired than patients with large-vessel VaD in the patterns of cognitive deficits.
