P089/268 Safety and efficacy of the fourth-generation Acandis Acclino Flex Plus stent in managing complex intracranial aneurysms
ConclusionThe AFP stent demonstrated feasibility and favourable safety and efficacy in the treatment of complex aneurysms. These results support its clinical use and highlight the need for further studies to evaluate its performance in different aneurysm subsets and to determine its specific role in the endovascular treatment of aneurysms.Disclosure of InterestCK serves as consultants for Acandis GmbH (Pforzheim, Germany) and proctor for MicroVention Inc./Sequent Medical (Aliso Viejo, CA, USA). P090/269 Comparative analysis between a second-generation (acandis acclino) and a first-generation (enterprise) stent system
ConclusionAdvanced stents offer reduced thromboembolic risk and comparable aneurysm occlusion to first-generation stents due to their flexible design and surface modification. These results support the clinical use of advanced stent systems, but further comparative studies are needed to conclusively evaluate the efficacy of stent-assisted coiling with the Acclino stent and other systems.Disclosure of InterestCK serves as consultants for Acandis GmbH (Pforzheim, Germany) and proctor for MicroVention Inc./Sequent Medical (Aliso Viejo, CA, USA). Mechanical thrombectomy is cost-effective versus medical management alone around Europe in patients with low ASPECTS
Conclusions MT is efficient versus MM alone for patients with low ASPECTS in eight countries across Europe. Patients with a large ischemic core could be treated with MT because it is both clinically beneficial and economically sustainable.

Collateral status and recanalization after endovascular treatment for acute ischemic stroke
Conclusions Collateral status is not associated with the probability of successful recanalization after EVT and does not modify the association between successful recanalization and functional outcome.

Benefit of successful reperfusion achieved by endovascular thrombectomy for patients with ischemic stroke and moderate pre-stroke disability (mRS 3): results from the MR CLEAN Registry
Conclusions Although patients with AIS with pre-stroke mRS 3 comprise a heterogenous group of disability causes, we observed improved outcomes when patients achieved successful reperfusion after EVT.

Endovascular treatment for isolated posterior cerebral artery occlusion stroke in the MR CLEAN registry
Conclusions EVT should be considered in selected patients with AIS with an isolated PCA occlusion, presenting with moderate–severe neurological deficits, as EVT was technically feasible in most of our patients and about half had good clinical outcome. In case of lower NIHSS score, a more conservative approach seems warranted, since periprocedural complications are not uncommon. Nonetheless, EVT seems reasonably safe considering the absence of sICH in our study.

P64 NIMBUS geometric clot extractor for tough clots: SPERO study results and clot composition
ConclusionsIn challenging cases with tough clot, first pass of NIMBUS retrieved clot richer in fibrin and lower in RBC content than clot retrieved with the first pass of standard MT devices.ReferencesFennell VS, Setlur Nagesh SV, Meess KM, Gutierrez L, James RH, Springer ME, Siddiqui AH. Quantified health and cost effects of faster endovascular treatment for large vessel ischemic stroke patients in the Netherlands
Conclusion One-minute faster EVT increases QALYs while cumulative costs remain largely unaffected. Therefore, faster EVT provides better value of care at no extra healthcare costs.

E-081 Susceptibility - Weighted imaging findings in experimental acute ischemic stroke model
ConclusionIn the hyperacute stages of MCAO, SWI images display a consistent and significant signal drop relative to the unaffected side. This signal drop is suspected to result from the paramagnetic effect of deoxyhemoglobin and venodilation and thus an indirect means of visualizing tissue hypoxia.Disclosures N. Saadat: None. G. E-045 Nihss and its component subscores are suboptimal predictors with colinearity
Conclusions NIHSS composite score is a suboptimal predictor for assessed patient outcomes. Principal component analysis improves predictive value of assessment with a statistically significant effect for pre-intervention patient outcomes analysis. Statistically significant variables are heavily dependent of NIHSS subscores, and variation in the distribution of these subscore may represent a significant source of bias. References .
