Futility, Miracles, and Religion: A Game Theory Display of a Medical Ethics Dilemma (P02.006)
CONCLUSIONS: Although this situation does not describe a Prisoner's Dilemma, that game theory format of payoff presentation demonstrates that ethical dilemmas in clinical medicine (such as this described situation involving futility and religious belief in miraculous healing) can be illustrated and the issues focused when displayed using a perceived payoff format.Disclosure: Dr. Riggs has nothing to disclose.

Determinants of White Matter Hyperintensity Burden in Patients with Fabry Disease (P03.171)
CONCLUSIONS: In FD subjects, the severity of cerebrovascular disease manifested by MRI-detectable WMH is greater than that of general population. WMH burden is associated with CMP in this cohort independently of age, and it may be affected by different disease processes in men and women and at different disease stages. Further studies exploring associations between WMHV, ERT, age- and gender-specific characteristics, as well as longitudinal WMH progression are warranted.Supported by: Genzyme, Sanofi Company; NIH-NINDS K23NS064052.Disclosure: Dr. Rost has received personal compensation for activities with Genzyme Corporatio...

Teaching NeuroImages: Concurrent hemifacial spasm and trigeminal neuralgia due to vertebrobasilar dolichoectasia
A 64-year-old man reported a 2-week worsening of a 2-year history of paroxysmal, lancinating right facial pain and spasms. Head CT demonstrated a tortuous and ectatic vertebral-basilar system. MRI confirmed a dolichoectatic basilar artery that distorted the right trigeminal (figure 1) and facial nerve roots (figure 2). He failed conservative medical management including carbamazepine, oxcarbazepine, and levetiracetam. He underwent Gamma Knife radiosurgery with few months of pain relief and was offered botulinum neurotoxin injection for spasms.

Reversible cerebral vasoconstriction syndrome and cervical artery dissection in 20 patients
Conclusion: The association of RCVS and CeAD was found in 12% of our patients with RCVS and 7% of our patients with CeAD. Underlying mechanisms are unknown. In practice, our results point to the need for a systematic study of both cervical and intracranial arteries in the 2 conditions.

Emerging Subspecialties in Neurology: Palliative care
As we work to find cures for so many devastating neurologic injuries and diseases, our patients suffer tremendously on a daily basis. Individuals with conditions including stroke, multiple sclerosis, Parkinson disease (PD), muscular dystrophies, amyotrophic lateral sclerosis (ALS), and nervous system malignancies share a host of physical, emotional, and existential symptoms that can be difficult to treat. In addition, patients and their families face the realities of loss of function, loss of ability to communicate, and lifespans limited by the neurologic diagnosis or complications related to it (e.g., dysphagia, immobilit...

An Unusual Cause of Altered Mental Status: Recurrent Cerebral Air Embolism from Atrial-Esophageal Fistula (P2.119)
Conclusion:The incidence of stroke secondary to air emboli is rare but a known complication of AEF's. Review of literature showed no reports of recurrent air embolism following successful treatment with HBO and repair of AEF. It is important to consider air embolism in any patient with a history of AEF, regardless of prior treatment.Disclosure: Dr. Ahmed has nothing to disclose. Dr. Myers has nothing to disclose. Reversible Cerebral Vasoconstriction Syndrome and Fingolimod: Culprit or Innocent Bystander (P2.224)
CONCLUSIONS:Reversible cerebral vasoconstriction syndrome may be a rare complication of Fingolimod treatment and should be considered in patients with new onset thunderclap headache. Quality of Life in Liver Transplant Patients with Neurologic Complications. (P4.031)
Conclusions: Our analysis shows that neurological complications worsen the quality of life of liver transplant patients. Especially regarding perception of health, ability to function independently due to physical and mental health limitations. Pain is a limiting condition in this group. Transplant patients who suffer neurological complications should be carefully followed up to reduce the impact on quality of life, in order to promote the ultimate goal of transplant.Disclosure: Dr. Gonzalez Toledo has nothing to disclose. Treatment of Post-Influenza Vaccination Induced Acute Disseminated Encephalomyelitis (ADEM) with Plasma Exchange - A Case Report (P4.036)
DISCUSSION:Influenza vaccination has been rarely linked to ADEM. Due to a low-incidence of the disease, there are no established treatment guidelines. Our patient did not respond to steroids but showed marked radiographic improvement with PE. Multiple Sclerosis Exacerbation Presenting as Stress Cardiomyopathy (P6.162)
Conclusions: Stress cardiomyopathy is increasingly suspected to be neurogenic in etiology. This case not only supports this theory, but also reports multiple sclerosis to be a potential cause of neurologic dysfunction leading to this cardiac phenomenon. In addition, it fortifies the theory that bilateral medullary lesions are required for many instances of autonomic dysfunction.Disclosure: Dr. Webb has nothing to disclose.
