Abstract 037: The Accuracy of Self-reported Blood Pressure in the Medication Adherence Improvement Support App for Engagement - Blood Pressure (MedISAFE-BP) Trial Session Title: Poster Session I
Discussion: In this cohort of individuals who reported that their BP was poorly controlled, only one-third actually had elevated BP when measured with a home BP cuff. While this discrepancy may have many underlying causes, it suggests that the use of self-reported BPs is not an accurate method of monitoring hypertension control at the population-level. Reassuringly, several factors are independently associated with accurate self-reported BPs, and thus there may be some subgroups for whom self-report can be relied upon.

Conclusions: The association of health insurance with CVD incidence varied by insurance group, and private insurance was associated with a lower risk of incident CVD. Further exploration of the features of health insurance coverage that impact CVD incidence may facilitate improvements in the primary prevention of CVD.

Abstract 150: Resource Utilization Patterns and Outcomes Among Acute Coronary Syndrome Patients: Findings From a Multi-Payer Analysis Poster Session II
Conclusion: These findings suggest that many patients with ACS do not receive appropriate recommended antithrombotic prophylaxis, and opportunities exist to improve therapy. Increased use of software tools such as AQuA may support enhanced education efforts aimed at improving adherence to guidelines and quality of care.

Abstract 228: Assessing Knowledge, Perceptions, and Cultural Attitudes Towards Stroke in Elderly Korean-Americans: The Strengthening Our Korean Elders Through Community-Partnered Education (STROKE-COPE) Project Poster Session II
Conclusions: Elderly Korean-Americans have substantial risk for stroke, but possess both language and belief barriers that may interfere with stroke prevention and response. A stroke education curriculum tailored for elderly Korean-Americans must address these culturally specific beliefs to be effective.

Abstract 235: Acute Care Measures of Rehabilitation Needs Associated with Discharge to Rehabilitation for Patients with Minor Stroke Poster Session II
Conclusions: Rehabilitation needs were documented for patients with minor stroke and almost one third were discharged to receive inpatient rehabilitation in an IRF or SNF. Taking pre-stroke disability into consideration for transition planning from acute care could facilitate earlier communication and planning with the next point of care.
