E-108 Endovascular treatment of patients with acute ischemic stroke due to underlying arterial dissection: a nationwide analysis
ConclusionsDespite a similar prevalence among patients admitted for AIS, CA dissections are associated with a higher rate of endovascular intervention and poorer short-term outcomes compared to VA dissections. This study provides essential epidemiologic and outcome data for the cerebrovascular neurosurgeon.Abstract E-108 Table 1Comparison of clinical characteristics and outcomes among patients presenting with acute ischemic stroke due to carotid or vertebral artery dissections Characteristic Vertebral Artery Dissection, N = 2,562 Carotid Artery Dissection, N = 2,789 p-value1 Age (years), Mean (SD) 53 (17) 57 (17) <0.00...

E-111 Repeat thrombectomy after large vessel occlusion stroke: incidence, clinical and technical outcomes
ConclusionsRepeat EVT for patients with re-occlusion within 30 days of thrombectomy is associated with favorable improvement in NIHSS, similar functional outcomes to patients undergoing single EVT, but has significantly higher risk of hemorrhagic conversion.Disclosures Y. E-113 Lower low-density lipoprotein level increases the risk of delayed parenchymal hematoma following endovascular thrombectomy
ConclusionLDL level <50 mg/dL independently predicted delayed PH following thrombectomy and LDL >100 was shown to be a protective factor, irrespective of statin use. Further study is needed to explore the underlying mechanism and identify the strategies to prevent post-thrombectomy delayed PH among patients with low LDL levels.Disclosures S. Ahn: None. S. E-126 Multicenter flow diversion in 200 patients for treatment of intracranial aneurysms
ConclusionThis report characterizes patient demographics, aneurysm characteristics, procedure complications and long-term angiographic and clinical outcomes for a large patient population across multiple medical centers treated with flow diversion. Overall, procedures resulted in a high rate of occlusion and low rate of serious complications. Continued iterative outcomes analysis is warranted particularly as flow diversion technology evolves. Despite diverse hospital settings, differences in presentation, patient characteristics and the inclusion of off-label treatments, flow diversion procedures remained safe and effectiv...

E-153 Mechanical thrombectomy for patients older than 80 years old with large vessel occlusion stroke: systematic review and meta-analysis
ConclusionOur results suggest that MT in the elderly population is effective to achieve functional independence. Our data was unable to provide information about 90-days mortality, all ICH and sICH. Given our limitations, a RCT comparing MT versus MM in this population would be needed to confirm our results. However, including elderly patients in RCTs is always challenging. So, our results could provide support in the real world to offer this highly effective treatment to this challenging population.Disclosures J. Sequeiros: None. M. Cornejo: None. F. Terry: None. C. E-173 Thrombectomy passes and clinical outcomes in patients with stroke intervention beyond 24 hours
ConclusionIn our patients undergoing MT beyond the 24-hour window, the number of device passes was not associated with functional outcome, successful recanalization, or sICH. This may be due to the delayed intervention window, small sample size, or other factors. A larger study and further investigation is warranted.Disclosures A. Pandhi: None. A. Alrohimi: None. M. E-189 Outcomes of tenecteplase versus alteplase after mechanical thrombectomy in a multiethnic stroke consortium registry
ConclusionIn this prospective, multi-center stroke consortium study, IV TNK in comparison to tPA prior to MT for ischemic stroke resulted in non-inferior outcomes related to angiographic reperfusion and functional status at discharge. These findings compliment the current literature and include a large Hispanic US cohort. Further analysis will include expansion of contributing centers and use of propensity scoring.Abstract E-189 Table 1 and 2Disclosures M. Gaub: None. D. Ramaswamy: None. A. Cardentey: None. S. E-197 Use of balloon-mounted stents and drug eluting stents in intracranial atherosclerotic disease: single center experience
ConclusionDES appears effective and relatively safe with good long-term outcomes in treating intracranial atherosclerosis and secondary stroke prevention and is associated with lower delayed in-stent stenosis on follow up imaging compared to BMS and Wingspan stents. Use of DES for symptomatic ICAD in acute and non-emergent settings warrants further investigation.Balloon mounted stent, drug eluting stent, Wingspan, intracranial atherosclerotic disease, outcomes.Disclosures N. E-211 Association of cerebrovascular injury and secondary vascular insult with poor outcomes following gunshot wound to the head in a large civilian population
ConclusionThese data suggest that intracranial CVI from GSWH and associated secondary vascular insult are associated with poor outcomes. Given the high prevalence and potentially reversible nature of these secondary injuries, early screening with vascular imaging and treatment of underlying cerebrovascular injury may prove critical to improve outcomes by reducing the rate of stroke and re-hemorrhage.Abstract E-211 Figure 1Disclosures J. E-231 Long-term safety and efficacy of the pipeline embolization device for treatment of small vs. large aneurysms: a systematic review and meta-analysis
ConclusionThe long-term occlusion rate after PED treatment is higher in large vs. small aneurysms. Symptomatic thromboembolic rates with stroke are also higher in large vs. small aneurysms. The difference in the rates of aneurysm rupture and intracranial hemorrhage was insignificant. Although the PED seems a safe and effective treatment for small and large aneurysms, further studies are required to clarify how occlusion rate and morbidity are affected by aneurysm size.Disclosures A. E-236 Automated pre- and post-operative volumes estimates risk of retreatment in chronic subdural hematoma: a retrospective, multi-center study
Conclusions/RelevanceLarger pre- and post-operative cSDH volumes increase the risk of cSDH retreatment. Volume thresholds may allow identification of patients at high risk of cSDH retreatment who would benefit from adjunct treatments. Machine learning algorithm can quickly provide accurate estimates of pre and post operative volumes.Disclosures J.
