Every 15‐min delay in recanalization by intra‐arterial therapy in acute ischemic stroke increases risk of poor outcome
ConclusionsLonger time to recanalization was associated with poorer functional outcome post intra‐arterial therapy. We recommend that a systematic approach to minimize time delay to treatment is warranted in intra‐arterial therapy for acute ischemic stroke.

Factors associated with six‐month outcome of pediatric stroke
ConclusionsFor patients surviving arterial ischemic stroke, the most significant clinical consequences both acutely and at six‐months, are sensorimotor impairments, particularly evident in the older children. In contrast cognitive or language sequelae were not identified. Long‐term surveillance is required to describe clinical course and rehabilitation needs, particularly for neonates and infants.

Association between atherogenic dyslipidemia and recurrent stroke risk in patients with different subtypes of ischemic stroke
ConclusionsAtherogenic dyslipidemia is associated with higher risk of stroke recurrence in ischemic stroke patients. Such association might be more pronounced in large‐artery atherosclerosis subtype and needs further investigation to establish such relationship.

Risk factors for ischemic stroke and its subtypes in Chinese vs. Caucasians: Systematic review and meta‐analysis
ConclusionOur study showed a lower prevalence of atrial fibrillation, ischemic heart disease and hypercholesterolemia in Chinese, and mostly similar risk factor associations in Chinese and Caucasian ischemic stroke patients. Further analyses of individual patient data to allow adjustment for confounders are needed to confirm and extend these findings.

Targeting aspirin in acute disabling ischemic stroke: an individual patient data meta‐analysis of three large randomized trials
ConclusionsUsing simple clinical variables to target aspirin to patients after acute disabling stroke by risk of thrombosis, haemorrhage or poor functional outcome does not lead to greater net clinical benefit. We suggest future risk stratification schemes include new risk factors for thrombosis and intracranial haemorrhage.

Sedation vs. Intubation for Endovascular Stroke TreAtment (SIESTA) – a randomized monocentric trial
ConclusionThe aims of this study are to prospectively clarify whether the non‐intubated state of conscious sedation is feasible, safe, and superior with regard to early neurological improvement compared to the intubated state of general anesthesia in patients receiving acute endovascular stroke treatment.

TALOS: a multicenter, randomized, double‐blind, placebo‐controlled trial to test the effects of citalopram in patients with acute stroke
DiscussionSSRI treatment is well tolerated and overall beneficial in the wake of stroke; it may also be neuroprotective and prevent new vascular events.

Burden of stroke in Italy: An economic model highlights savings arising from reduced disability following thrombolysis
ConclusionOur study reveals that performing thrombolytic therapy in eligible patients improves economic outcomes compared with patients receiving standard care. This model is useful for decision makers, both within and outside of the Italian national context, as a tool to assess the cost‐effectiveness of thrombolysis in both short‐ and long‐term period.

National Institutes of Health Stroke Scale score is an unreliable predictor of perfusion deficits in acute stroke
ConclusionsAlthough higher National Institutes of Health Stroke Scale scores are predictive of perfusion deficits, many patients with no clinically detectable signs have persisting cerebral blood flow changes. A National Institutes of Health Stroke Scale score threshold should therefore not be used to select patients for perfusion‐weighted magnetic resonance imaging. Perfusion‐weighted magnetic resonance imaging should be considered in all patients presenting with acute focal neurological deficits, even if these deficits are transient.

Rationale, design, and progress of the ENhanced Control of Hypertension ANd Thrombolysis strokE stuDy (ENCHANTED) trial: An international multicenter 2 × 2 quasi‐factorial randomized controlled trial of low‐ vs. standard‐dose rt‐PA and early intensive vs. guideline‐recommended blood pressure lowering in patients with acute ischaemic stroke eligible for thrombolysis treatment
ConclusionsLow‐dose iv rt‐PA and early intensive BP lowering could provide more affordable and safer use of thrombolysis treatment for patients with AIS worldwide.

ICARUSS, the Integrated Care for the Reduction of Secondary Stroke trial: rationale and design of a randomized controlled trial of a multimodal intervention to prevent recurrent stroke in patients with a recent cerebrovascular event, ACTRN = 12611000264987
ConclusionThe ICARUSS study aims to recruit and follow up patients between 2007 and 2013 and demonstrate the effectiveness of exposure to the ICARUSS model in stroke survivors to reduce recurrent stroke or vascular events and promote the implementation of best practice risk factor management at primary care level.

Antithrombotic treatment at onset of stroke with atrial fibrillation, functional outcome, and fatality: a systematic review and meta‐analysis
ConclusionsTherapeutic International Normalized Ratio at stroke onset was associated with early and late improved survival and functional recovery suggesting sustained benefit for warfarin anticoagulation for stroke outcome in atrial fibrillation patients. Long‐term outcome data following stroke in patients taking new oral anticoagulants is required.

Solitaire™ with the Intention for Thrombectomy as Primary Endovascular Treatment for Acute Ischemic Stroke (SWIFT PRIME) trial: protocol for a randomized, controlled, multicenter study comparing the Solitaire revascularization device with IV tPA with IV tPA alone in acute ischemic stroke
RationaleEarly reperfusion in patients experiencing acute ischemic stroke is critical, especially for patients with large vessel occlusion who have poor prognosis without revascularization. Solitaire™ stent retriever devices have been shown to immediately restore vascular perfusion safely, rapidly, and effectively in acute ischemic stroke patients with large vessel occlusions. AimThe aim of the study was to demonstrate that, among patients with large vessel, anterior circulation occlusion who have received intravenous tissue plasminogen activator, treatment with Solitaire revascularization devices reduces degree of disab...
