Abstract 223: Patients With Cardiac Comorbidities Carry Worse Outcome as Identified by Our SELF Risk Stratification Pathway: An ACAP-SELF Syncope Dataset Analysis Session Title: Poster Session II
Conclusions: Using the SELF-pathway for patients presenting with syncope effectively identifies high risk patients who merit hospitalization and close follow-up post-discharge. These include patients with structural heart disease, abnormal EKG and abnormal telemetry, as well as patients with diabetes, CAD and CHF. This has important implications for the evaluation of a common disease that poses a significant economic burden on healthcare systems.

Abstract 304: Nonfatal Outcomes Following Incident Atrial Fibrillation: A Community Study Session Title: Poster Session III
Conclusions: In the community, adverse outcomes are frequent after AF, and HF occurs more frequently than IS/TIA. Risk factors differed for IS/TIA and HF, suggesting that pathways to the occurrence of these outcomes, as well as prevention strategies, may differ. Finally, the occurrence of these outcomes did not decline over the last decade, highlighting the importance of continued rigorous surveillance of adverse outcomes in AF.

Abstract 351: Optimizing the Frequency of Surveillance Echocardiography in Clinically Stable Patients With Hypertrophic Cardiomyopathy Session Title: Poster Session III
Conclusions: In select clinically stable patients with HCM, repeat echo may be unlikely to identify significant changes or impact practice. In these patients, a decreased frequency of echocardiography could decrease resource utilization without compromising care. Ongoing data collection and better risk stratification tools may allow more targeted surveillance strategies in the future.

Conclusions: Symptom type and characteristic, and cognitive behavioral responses of early treatment seekers differed from those of late treatment seekers. Experiencing a rapid heartbeat and unremitting symptoms may have helped participants form a cognitive appraisal and behavioral response conducive to early treatment-seeking. Experiencing intermittent symptoms less identifiable with a cardiac origin may have hindered development of cognitive appraisals and behavioral responses that promote early treatment- seeking. Conclusions: AA who are admitted with unexplained syncope are identified according to the SELF pathway as high risk group that can likely be attributed to the presence of comorbidities and warrants hospitalization for further work-up and optimizing medical management.

Conclusions— AF is common clinical problem with significant morbidity and mortality. Get With The Guidelines-AFIB is a national hospital-based AF quality improvement program designed to increase adherence to evidence-based guidelines for AF.

Conclusions: As heart failure progresses, transfer to palliative or hospice services is often appropriate. Including patients when death is imminent in outcome analyses could overestimate medication effects. Mechanisms for identifying such patients other than hospice use are limited but constitute an important goal for future research.

Abstract 119: Daily Cardiac Catheterization Procedural Volume and Complications Session Title: Poster Session I
Conclusions: There is a U-shaped association between CC volume and rates of CC related complications. Unlike prior studies which identified high annual volumes were associated with lower complication rate, our study results suggests that this relationship does not hold true for daily volume. There appears to be a daily limit at which complication rates begin to rise in proportion to volume. Higher complication rates were seen on days with very low CC volume (<5 procedures) and days with very high CC volume (>12 procedures). Conclusion: Persistence rates for warfarin were higher at one year than those on dabigatran. In addition, factors associated with persistence of warfarin include: African American and Hispanic race, type of AF including permanent and persistent, LVH, and CHA2DS2-VASc risk scores ≥ 2.

Conclusion: Traditional HF therapies are commonly prescribed to LVAD recipients, although less frequently than to advanced HF patients without LVAD support. Aldosterone antagonists are prescribed increasingly to LVAD patients. Further research is needed on the optimal medical regimen for patients with LVADs.
