Patients with atrial fibrillation (afib) have a high rate of serious complications including stroke and decompensated heart failure. While patients with afib are five times more likely to suffer a stroke in their lifetime than the general population, this risk can be reduced by 64% with appropriate anticoagulation using warfarin or approved novel oral anticoagulants (NOACs). Abstract 22: Impact of CHA2DS2-VASc Risk Factors on Anticoagulant Prescription in Patients with Atrial Fibrillation: Insights From the NCDR(R) PINNACLE Registry Session Title: Abstract Oral Session
Conclusions: Among this cohort of AF patients with an indication for OAC use, older age and HTN were strongly associated with greater OAC use whereas female gender and vascular disease were associated with less OAC use. Further investigation is needed to understand reasons for these differences in how risk factors influence decisions to provide OAC, such as patient or provider preference or gender bias.

Abstract 152: Persistence of Warfarin versus Dabigatran Therapy in Patients with Atrial Fibrillation: Results from the ORBIT AF Registry Session Title: Poster Session I
Conclusion: Persistence rates for warfarin were higher at one year than those on dabigatran. In addition, factors associated with persistence of warfarin include: African American and Hispanic race, type of AF including permanent and persistent, LVH, and CHA2DS2-VASc risk scores ≥ 2.

Conclusion: AF is tough to diagnose since it is often asymptomatic and clinicians need many tests to confirm diagnosis. Lack of documentation or detection of AF and its associated conditions is a concern. AF can lead to life threatening problems such as stroke and heart failure. The surveillance of AF in India is important in order to identify opportunities for intervention. Continued data collection through the use of a cardiovascular registry can improve the understanding of AF in India.

Risks and Benefits of Anticoagulation in Atrial Fibrillation: Insights From the Outcomes Registry for Better Informed Treatment of Atrial Fibrillation (ORBIT-AF) Registry Original Articles
Conclusions— In community-based outpatients with AF, use of OAC was high and driven by not only predominantly stroke but also bleeding risk. Stroke risk significantly affects OAC use among those with low bleeding risk, whereas those with high bleeding risk demonstrate consistently lower use of OAC regardless of stroke risk.

Abstract 25: Predicting Outcomes among Patients with Heart Failure Receiving Anticoagulation with Warfarin Concurrent III Session A: Oral Abstract Presentations on EHS & Health IT
Conclusion: We developed a composite score for CHF severity which predicted both anticoagulation control and the rate of major hemorrhage among patients anticoagulated with warfarin. This study suggests that relatively easily observable characteristics can be used to risk-stratify patients with CHF who are receiving anticoagulation for AF.

Abstract 217: How well does the CHADS2 Stroke Risk Score Predict Major Hemorrhage in Patients with Atrial Fibrillation? Poster Session II
Conclusions: Although some clinical markers of stroke risk such as those included in the CHADS2 risk score are also associated with increased hemorrhage risk, the CHADS2 risk score should not be used instead of a validated hemorrhage risk tool to estimate hemorrhage risk.

Abstract 227: Prescribing Warfarin at Discharge for Heart Failure Patients without Atrial Fibrillation Findings from the Get With The Guidelines-Heart Failure Registry Poster Session II
Conclusions: Approximately 1 in 10 HF patients without AF or other documented indications are prescribed warfarin at discharge, and prescription rates vary widely across hospitals. Further research is needed to understand why anticoagulation is prescribed to patients with HF in the absence of documented indications.
