P-004 Outcomes Stratified by nature of deficits at presentation (Disabling versus Non-disabling) in low NIHSS LVO patients undergoing mechanical thrombectomy
ConclusionsOur retrospective study demonstrates a trend towards better functional outcomes in LVO patients with low NIHSS who were considered to have disabling deficits compared to those with non-disabling presenting symptoms. Future multicenter studies with a larger sample size and/or ongoing randomized trials for this subpopulation of LVO patients should examine this hypothesis further.Disclosures N. Goyal: None. J. E-049 Complications of middle meningeal artery embolization - a systematic review
ConclusionThe published literature suggests that MMA embolization is a generally well-tolerated procedure, but with a low risk of significant complications, including, but not limited to stroke, seizures, visual obscurations, and facial palsy. E-125 Gaze weakness neglect and speech (GWNS): an acute ischemic stroke scale of large vessel occlusion (LVO) in the emergency department for faster treatment
ConclusionsGWNS is an AISS of LVO which could be performed rapidly in ER. GWNS score 3 and 4 are associated with LVO, especially when gage deviation/preference are present. Additionally, GWNS scale was superior to NIHSS to detect LVO. Therefore, patients with AIS with GWNS score 3 or 4 could be directly send for early MT which will save time and millions of brain cells. E-235 Automated quantification of arcuate, superior longitudinal, and inferior longitudinal fasciculi using diffusion tensor tractography correlates with speech function in post-thrombectomy stroke patients
ConclusionFor left-sided MCA occlusions, TICI scores were well correlated with increased arcuate and superior longitudinal fasciculi tract counts after MT. After recanalization of the left MCA, increased number of arcuate, superior longitudinal, and inferior longitudinal fasciculi tracts is associated with improved speech function measured by the WAB test. These two findings were not apparent in right-sided MCA occlusions, perhaps due to the minimal role of the right cerebral hemisphere in speech. Performance of the vision, aphasia, neglect (VAN) assessment within a single large EMS system
Conclusions Among suspected stroke patients, we found modest sensitivity and specificity of VAN to detect LVO for both EMS and ED providers. Moreover, the low PPV in our study suggests a significant number of patients with non-LVO ischemic stroke or ICH could be over-triaged with VAN.

Monitored anesthesia care during mechanical thrombectomy for stroke: need for data-driven and individualized decisions
Conclusions Our study showed that the overall rate of conversion from MAC to GA during MT was low (1.6%) and, while higher in posterior circulation strokes, it was not predicted by either hemispheric dominance or stroke severity. Caution should be given before changing clinical practice during moments of crisis.

E-057 Prospective prehospital evaluation of VAN and RACE large vessel occlusion prediction instruments on a mobile stroke unit
ConclusionsBoth VAN and RACE demonstrated measurable but limited benefit in predicting LVO in the field. In patients with LKW times of ≤1.5 hours, RACE showed a high NPV of 0.96. In this subset of patients, RACE could be used to rule out LVO in the prehospital setting, avoiding delays in care due to unnecessary transport to a thrombectomy-capable facility.Disclosures J. Too good to intervene? Thrombectomy for large vessel occlusion strokes with minimal symptoms: an intention-to-treat analysis
Conclusions Thrombectomy led to a shift towards a lower NIHSS in patients with LVO presenting with minimal stroke symptoms. Despite the overall perception that this condition is benign, nearly a quarter of patients primarily treated with medical therapy did not achieve independence at 90 days.

Conclusion In this large cohort, the VAN screening tool has been validated and identified LVO more accurately than any other screening tool. Stroke vision, aphasia, neglect (VAN) assessment--a novel emergent large vessel occlusion screening tool: pilot study and comparison with current clinical severity indices
Conclusions The VAN screening tool accurately identified ELVO patients and outperformed a NIHSS ≥6 severity threshold and may best allow clinical teams to expedite care and mobilize resources for ELVO patients. A larger study to both validate this screening tool and compare with others is warranted.
