Emerging role of various signaling pathways in the pathogenesis and therapeutics of atherosclerosis
Publication date: December 2017 Source:Reviews in Vascular Medicine, Volumes 10–11 Author(s): Yash Prashar, Ritu, Souravh Bais, Naresh Singh Gill Atherosclerosis is a leading cause of mortality and morbidity in the western world. It is no longer a disease attributed mainly to the high lipid content of the body but has come to be regarded as a chronic inflammatory disease with an autoimmune component. Studies which explore the interactions between molecular and cellular elements generally focus on pathophysiologic aspect of atherosclerosis. The focus has now shifted to the novel risk factors and the genetic predispo...

Placenta percreta with iatrogenic megaureter: A maternal near miss case presentation
Conclusions A decision between radical and conservative strategies for placenta accreta must be made based on the degree of placental infiltration and other variables: the patient's hemodynamic status and her desire to remain fertile. In our opinion, cesarean hysterectomy remains the best therapeutic option to treat placenta percreta. Radical surgery should be done for poor availability of blood especially in rare blood types in other types of placenta accreta. Fullness in the retroperitoneal space should not be ligated except after excluding ureter dilatation and confirming hematoma by aspiration and disscetion. Megaurete...

Placental polyps with uterine vascular malformation mimics: Management dilemma
Conclusions Uterine vascular malformations may be associated with heavy bleeding. They should be suspected on ultrasound with anechoic structures with positive Doppler signal. Dilatation and curettage is therapeutic for evacuation of placental polyps but can induce massive, life-threatening bleeding in cases with uterine arteriovenous shunting or malfomration (AVM), thus it should be done with caution in endometrial pathology with high velocity flow more than 40cm/s as infection and adhesions are associated. Treatment of uterine AVM varies from medical management (hormonal therapy), minimally invasive uterine artery embol...

Anterior and posterior Placenta previa percreta in blood group 0 negative patient with urologic complications: An interesting maternal near miss case presentation
Conclusions A decision between radical and conservative strategies for placenta increta and percreta must be made based on the degree of placental infiltration (whether it goes beyond the perimetrium and into adjacent organs) and other variables: the patient’ s hemodynamic status and her desire to remain fertile. In our opinion, cesarean hysterectomy when the perimetrium is intact remains the best therapeutic option to treat placenta increta and percreta. If, however, placental invasion largely exceeds the perimetrium and/or the patient wishes to remain fertile, management of the placenta in situ is indicated, with full ...

Postpartum purpura fulminans: an unusual presentation complicated with amputation of limbs
This article reports a case of Purpura Fulminans presented with DIC after hemorrhagic shock of postpartum hemorrhage, treated at our centre with review of etiology, pathogenesis, clinical features and treatment. S. aureus infection was diagnosed on the basis of culture results, and susceptibility to methicillin was determined. The strain was isolated from the blood cultures. Staphylococcal purpura fulminans may be a newly emerging illness associated with superantigen production. Medical practitioners should be aware of this illness. (Source: Reviews in Vascular Medicine)

Cutaneous small vessel vasculitis of the lower legs
Publication date: Available online 11 April 2017 Source:Reviews in Vascular Medicine Author(s): Massimo Papi Cutaneous small vessel vasculitis (CSVV) are a spectrum of conditions characterized by an angiocentric inflammation mainly limited to the post-capillary venules of the skin. Neutrophils are the predominant cellular effectors of the inflammatory infiltration and destruction of the blood vessels. The most common clinical presentation is palpable purpura of the lower half of the legs but type, site and calibre of the involved vessel district influence the localization and severity of the clinical features that may i...

Identification of vein graft stenosis and assessment of sustainability of outcomes: two sides of the same coin in vein graft surveillance
Publication date: Available online 11 April 2017 Source:Reviews in Vascular Medicine Author(s): Matthew Thomas, Reza Mofidi Autologous veins are the conduit of choice when performing an infra-inguinal arterial bypass procedure. However these grafts are at risk of failure. This is multifactorial in nature and relates to patient factors, factors relating to the procedure as well as adaptive processes which occur after the formation of an infainguinal bypass graft. Duplex ultrasound assessment of haemodynamic findings within the graft is the most accurate method of identifying vein graft stenosis and identifying grafts wh...

Placental polyps and acquired uterine arteriovenous malformations: management dilemma
Conclusions Dilatation and curettage is therapeutic for evacuation of RPOC and non-invasive hydatiform mole, but can induce massive, life-threatening bleeding in AVM, thus contraindicated in cases of high velocity flow more than 20cm /sec. Treatment of uterine AVM varies from medical management (hormonal therapy), minimally invasive uterine artery embolization to more definitive surgical hysterectomy, depending upon age of the patient, size and site of the lesion, and the desire to retain future fertility [5]. Etiology and mechanisms of age-related arterial stiffening
Publication date: Available online 8 December 2016 Source:Reviews in Vascular Medicine Author(s): Nabil E. Boutagy, Timothy J. Werner Cardiovascular disease (CVD) is the leading cause of death in the United States (U.S.) and its prevalence increases with advancing age1, 2. It is estimated that 83.6 million people in the U.S. have CVD2. As such, there is a high economic cost ($414 billion) associated with this disease1. In the past decade, large-scale epidemiological studies have identified a key role of large artery stiffening in the etiology CVD3-5. Stiffening of the large elastic arteries, in particular the aorta, is...

Modeling Vasomotion
Publication date: Available online 18 November 2016 Source:Reviews in Vascular Medicine Author(s): A. Fasano, A. Farina, A. Caggiati The phenomenon of vasomotion, consisting in periodic oscillations of blood vessels walls is particularly important at the scale of small vessels and exhibits different features in arterioles (in which flow is mainly driven by the hydraulic pressure gradient) and in venules provided with valves preventing back flow and helping centripetal hematic propulsion. Here we formulate a model for both situations, based on approximations of the flow equations implied by the smallness of the radius-...
