Inappropriate ICD shock due to hot tub-induced external electrical interference
A 72-year-old white male with a history of rapid nonsustained ventricular tachycardia, hypertrophic cardiomyopathy, and intermittent Brugada-type ECG had a single-lead implantable cardioverter-defibrillator (ICD) implantation and received a sudden ICD shock while in the hot tub. Electrocardiographic changes as a marker of improved subendocardial ischemia in a patient with hypertrophic cardiomyopathy
We report a case of non-obstructive HCM, in which electrocardiographic changes were observed with improvement of subendocardial ischemia. A 76-year-old man presented with chest pain on exertion. The electrocardiogram revealed left ventricular (LV) hypertrophy with repolarization abnormalities. No coronary stenosis was found on computed tomography angiography, but thallium-201 exercise scintigraphy revealed transient LV cavity dilation after exercise, consistent with subendocardial ischemia. Fully automatic detection of strict left bundle branch block
Strict left bundle branch block (tLBBB) is a diagnostic marker that was proposed for the detection of complete LBBB from ECG. The criteria for tLBBB include the presence of QS- or rS-configurations of QRS in V1 and V2, the presence of mid-QRS notching or slurring in at least two of leads V1, V2, V5, V6, I and avL, and finally a QRS duration (QRSd) of>130  ms (women) or 140 ms (men). Application of wireless remote electrocardiogram monitoring device in atrial fibrillation patients undergoing radiofrequency catheter ablation
Radiofrequency catheter ablation (RFCA) has emerged as an important therapeutic approach for atrial fibrillation (AF) patients [1]. In patients undergoing RFCA, recurrent AF is common in the first 3  months after the procedure [2]; hence, patients are maintained on anticoagulant drugs during this period. Although most studies have defined successful RFCA as cessation of symptomatic arrhythmia episodes, several studies have shown that asymptomatic episodes of AF are common in patients even aft er RFCA [3]. Wide QRS complex tachycardia in patients with Chagas ’ disease: Sustained ventricular tachycardia until proven otherwise
We describe a case of a patient with positive serology for Chagas disease and normal echocardiogram who presented an episode of wide QRS complex tachycardia. It was initially diagnosed as sustained ventricular tachycardia using the Brugada and Vereckei algorithms and it was sent to the reference service for the implant of a cardioverter defibrillator.
