Perioperative echocardiographic assessment of mitral valve regurgitation: a comprehensive review [REVIEWS]
Cardiac ultrasound allows comprehensive analysis of the complex structure of the mitral valve. Transoesophageal echocardiography is the preferred diagnostic and monitoring technique both in the intraoperative phase and in the immediate postoperative phase. Besides two-dimensional imaging, the 3D approach offers clear advantages in visualizing different aspects of pathology and facilitating interpretation of pathophysiological features. This review comprehensively summarizes anatomical and physiological characteristics, the value of 2D imaging in diagnosis and follow-up and the supplementary significance of 3D echocardiogra...

Cardiogenic shock induced by a voluminous phaeochromocytoma rescued by concomitant extracorporeal life support and open left adrenalectomy [CASE REPORT]
We report the case of a 57-year old patient presenting a voluminous phaeochromocytoma of the left adrenal gland revealed by cardiogenic shock and rescued by peripheral extracorporeal life support device (ECLS). Left surrenalectomy has been performed under protective ECLS during the same surgical procedure. The patient was successfully weaned from ECLS on Day 7 after surgery and myocardial function completely recovered when the patient was discharged from the hospital. Risk of postoperative complications in chronic obstructive lung diseases patients considered fit for lung cancer surgery: beyond oxygen consumption [THORACIC]
CONCLUSIONS VO2 is not the unique parameter to consider when CPET is performed to evaluate the postoperative risk of lung cancer surgery in COPD patients. The signs of ventilatory inefficiency such as VE/VCO2 slope predict complications better than VO2 does. (Source: European Journal of Cardio-Thoracic Surgery)

Comparison of outcomes between neuroendocrine thymic tumours and other subtypes of thymic carcinomas: a joint analysis of the European Society of Thoracic Surgeons and the International Thymic Malignancy Interest Group [THORACIC]
CONCLUSIONS This represents the largest clinical series of TCs and NETTs collected. Despite the biological aggressiveness of these rare neoplasms, the 5-year survival rate after resection is over 60% and TCs and NETT showed a similar rate of survival and recurrences after surgery. Psf3 is a prognostic biomarker in lung adenocarcinoma: a larger trial using tissue microarrays of 864 consecutive resections [THORACIC]
CONCLUSIONS The present findings strengthened our previous data demonstrating that high Psf3 expression in primary lung adenocarcinoma plays an important role in disease progression and is a prognostic indicator, particularly in early-stage adenocarcinoma. Exercise capacity in ventricular assist device patients: clinical relevance of pump speed and power [TRANSPLANTATION AND MECHANICAL CIRCULATORY SUPPORT]
CONCLUSIONS Although VAD speed increase provided an additional pump flow of 1.6 l/min at peak exercise, no significant objective and subjective benefits on patients' exercise performance were observed. Finally, VAD power could be a useful parameter to monitor patients during exercise. (Source: European Journal of Cardio-Thoracic Surgery)
