Analysis of Revenue, Expense, and Profitability in Surgical Procedures

The recent Journal of the American College of Surgeons article by Rosemurgy and colleagues1 entitled, “Cost Analysis of Pancreaticoduodenectomy at a High-Volume Robotic Hepatopancreaticobiliary Surgery Program,” tackles the difficult issue of evaluating profit from surgical procedures. The authors reconciled global payment for nonprofessional services with the cost of performing either robotic o r open pancreaticoduodenectomy to determine payor-specific profitability. As would be anticipated, there was significant variability in the payments received, with profitability in patients undergoing robotic pancreaticoduodenectomy achieved in less than one-third of patients and only 1 of 35 patien ts covered by Medicare.
