How to Prevent Harm When Performing Outpatient Procedures

After several RCAs involving unintentionally retained foreign objects (URFOs) and wrong site surgeries (WSS) in the outpatient setting, a multidisciplinary working group was assembled to determine corrective actions. Root Cause Analysis (RCA) results were compared, and several areas needing improvement were consistently identified as causal factors of these events: Communication, accountability, and teamwork.
