Emergency Department Weight Accuracy in Treatment of Deep Vein Thrombosis and Pulmonary Embolism with Intravenous Heparin
Intravenous heparin is a high-alert weight-based medication used in the treatment of deep vein thrombosis and/or pulmonary embolism. Inaccurate weight can lead to under or over dosing errors and potentiate negative patient outcomes. Emergency department (ED) weight measurement is obtained by nurse visual weight estimation or patient self-report.
