The future of NHS patient safety investigation

It is seeking views on how the ' Serious incident framework ' (2015) could be revised to support the system to respond appropriately when things go wrong. A discussion document has been produced which should be read before completing the online questionnaire. The engagement is open until 12 June 2018. It is intended to published the new framework by the end of 2018.
