Collecting social determinants of health (SDOH) information from patients is challenging. It is not easy to ask if a person is experience food insecurity or is currently unhoused. QliqSOFT is making it easier to get this important information while maintaining dignity. One of the main goals of this model is to “Advance health equity to bring the benefits of accountable care to underserved communities.” “Collecting the SDOH data necessary for ACO REACH can be awkward and challenging,” said Weber. “What we are finding is that patients are much more comfortable providing the information through a chatbot rather than in a conversation with a clinician.” QliqSOFT sends the questions to the patient as a link to a chatbot which asks the needed questions via a mobile or online interface.
