Navigating Population Health Data and HEDIS in an Ambulatory Practice

One of the big challenges ambulatory practices face today is how to navigate the increasingly complicated world of population including things like HEDIS scores.  The good news is that the opportunity to do population health is now there thanks to all of the data that’s available to a healthcare clinic in their EHR.  However, it takes more than just a report to do population health effectively and to ensure their closing the gaps in care. A few years ago. eClinicalWorks introduced a data analysis tool called HEDIS Analytics that brings to the surface gaps in care among a clinical practice’s population.  This tool is a customized way for practices to be able to effectively do population health in their organization.  Plus, since it’s integrated right in the eCW EHR, even clinicians at the point of care can access the population health tab to see any areas that can be addressed with the patient during the visit.
