Obviously, I can't speak from a place of personal experience, but I've spoken to a lot of doctors over the years and regularly browse medical forums. To answer your question, it's known to get much better once you start specialising, but again this is variable depending on where you train and what department you go into. Some specialities are known for having a better work-life balance e.g. Radiology, Dermatology, GP (although that's not to say that these jobs aren't still difficult) whereas others have a reputation for not e.g. ED.
