Image Case Report: Glandular Swelling
The Case A 47-year-old woman presented with a one-year history of bilateral submandibular gland swelling, mild symptoms of xerostomia and xerophthalmia and arthralgias in her fingers. A review of systems was otherwise unremarkable. On physical examination, her submandibular glands on both sides were enlarged and had a firm texture. Her parotid glands were normal, as... Mesenchymal stem cell-derived HGF attenuates radiation-induced senescence in salivary glands via compensatory proliferation
Head and neck cancer accounts for more than 550 000 cases annually worldwide [1] of with 70% of patients receiving radiotherapy. A common severe side effect of radiotherapy is hyposalivation and a consequent dry mouth sensation (xerostomia) which is often very difficult to treat [2]. The major cause of this side effect is the often unavoidable exposure of the salivary glands (SG) to radiation due to their position relative to the tumor within the radiotherapy treatment plan. Treatment planning comparison for head and neck cancer between photon, proton, and combined proton-photon therapy - from a fixed beam line to an arc
CONCLUSION: CPPT for HNC needs adaptive treatments. Increasing proton beam flexibility in CPPT, either by using a gantry or an upright-positioned patient, improves treatment plan quality. However, the photon component is substantially reduced, therefore, the balance between improved plan quality and costs must be further determined.PMID:37913953 | DOI:10.1016/j.radonc.2023.109973 (Source: Radiotherapy and Oncology : journal of the European Society for Therapeutic Radiology and Oncology)

Unusual Gingival Presentation Following Treatment With Pembrolizumab: A Case Report
Immune checkpoint PD-1 inhibitors such as pembrolizumab plus gemcitabine, vinorelbine, and liposomal doxorubicin (pembro-GVD) has demonstrated efficacy in second-line therapy for relapsed/refractory classical Hodgkin lymphoma (cHL). Although generally well tolerated, pembrolizumab could lead to immune-related adverse events (irAEs) such as rash, fatigue, hyperthyroidism, pneumonitis, hepatitis, and colitis. Oral irAEs are not uncommon among immune checkpoint inhibitors which include xerostomia, dysgeusia, lichenoid reactions and mucositis.
