Stimulant/Naltrexone for Weight
I've had some patients that benefitted from bupropion/naltrexone for weight loss. I got to wondering about my ADHD patients that are on methylphenidate or amphetamine (but not bupropion) if adding naltrexone to their regimen would be helpful for weight loss. Does anyone have experience with, or thoughts about, this? (Source: Student Doctor Network)

Risperdal withdrawal
I have a client with bipolar 1 disorder who has taken 6mg of risperdal daily for years. The medication has worked well for her but led to an 80 pound weight gain. The client has been working with weight management and taking Wegovy for 5 months with no weight loss at all. She quit her risperdal cold Turkey more than a week ago. Today, I noticed irritability and restlessness. The client has started having sleep difficulties. I reflected my observations today and expressed concerns for her... Sorry some dumb medical questions from a PGY1
I have some really dumb questions, if anyone would be willing to answer them! 1. Do you hold all GDMT or just the beta blocker during heart failure exacerbation? What about Farxiga? Is Amlodipine or Nifedipine contraindicated in HFrEF/HFpEF? 2. Are there any formal guidelines on when to start pharmacology for weight loss, such as Phentermine-topiramate? 3. How do you know if a chest tube is currently to suction, on water seal, or clamped? 4. How exactly do you diagnose DKA? Do you need... Read more (Source: Student Doctor Network)

Question on weight-loss drug dosage.
Gettings, We have a patient who gets Phentermine 37.5 mg BID # 60 each month. My question is, is 75 mg/ day justified? I can’t find it anywhere in package insert to go above 37.5 mg (although we have some other folks on 1.5 tab of 37.5 mg qd). Thanks (Source: Student Doctor Network)

Anyone here who started their own private practice?
How did you get patients? Did you send mailers? Just word of mouth? I’m basically about to start my concierge weight loss clinic. Have only spent maybe $1500 so far and have website 80 percent done, emr is set up with full tele med capability, lab portal is set up. But once i got a quote for malpractice insurance, I’m starting to get cold feet. The quote was for $5600, and that’s in fact for part time practice. And it’s a claims made policy so if i fail at the business or if it’s not... Read more (Source: Student Doctor Network)

Topiramate
Curious what everyone’s experience has been with topiramate and it’s many many off label uses. I had a conversation with someone the other day who frequently used it for weight loss, mdd, ocd, sleep, Alcohol deprenence and other off label uses. I’ve never really used it because in my minds eye there’s always something that is “better“ in each given category of use and frankly I’m just not very familiar with it. What has the group seen by way of efficacy in all of its off label uses? My... Read more (Source: Student Doctor Network)

Glp-1 agonists
Articles about using ozempic etc for weight loss have been all over the popular press. Has anyone had a patient using a glp-1 agonist inappropriately as part of eating disordered behaviors? I figure it's only a matter of time but I haven't come accross it yet. (Source: Student Doctor Network)

Outpatient PA supervisor question
I wasn’t sure how to title this.. There is a PA I work with who has an outpatient suboxone and testosterone clinic basically, he started doing ozempic for weight loss recently as well. He has a retired ER doc that started the practice and now just has his name on it for legal purposes. His current doc is retiring from everything and I was approached to take his place. I’m interested because I’m getting burned out and would like to do some outpatient medicine. I’ve been considering ketamine... Read more (Source: Student Doctor Network)
