A Case with Cobalamin-C-Deficiency and Severe Heart Failure Resolved by LVAD Implantation and Subsequent Heart Transplantation
Introduction: Cobalamin c deficiency (cblC), an inborn error of vitamin B12 metabolism, is caused by mutations of the MMACHC gene. Patients exhibit multisystem involvement with as many as 50% afflicted with various structural heart defects. Severe congestive heart failure (HF) may also occur with a poorly documented clinical course. (Source: The Journal of Heart and Lung Transplantation)

Oral Vitamin B12 Versus Intra-Nasal Vitamin B12 in Transient Neurosensory Disturbances Following Third Molar Surgery: A Prospective Cohort Study
ConclusionIntra-nasal vitamin B12 sprays were found to be conducive adjuvants to treat transient neurosensory disturbances following third molar surgery. Further studies on its routine application post-neurosensory complications are recommended. (Source: Journal of Maxillofacial and Oral Surgery)

The Alarming Rate of Malnutrition after Single Anastomosis Sleeve Ileal Bypass. A single Centre Experience
ConclusionDespite good short-term weight loss and improvement of obesity-associated complications, SASI is accompanied by high alarming malnutrition, even in short-term follow-up. Novel MBS should be judged for their long-term effects and compared to well-tested standard operations before they are used in routine clinical practice.Graphical Abstract (Source: Obesity Surgery)

Recurrent venous thrombosis - an unusual first presentation of autoimmune polyendocrinopathy syndrome type 3B
S Afr Med J. 2023 Dec 31;114(1):17-21. doi: 10.7196/SAMJ.2024.v114i1.1477.ABSTRACTA 45-year-old female presented with unprovoked recurrent venous thromboembolism (VTE), in unusual sites, and pancytopenia, posing a complex diagnostic challenge. Work-up for inherited thrombophilia, antiphospholipid syndrome (APLS) and paroxysmal nocturnal haemoglobinuria were unremarkable. Investigations revealed autoimmune thyroid disease, and a mixed iron/vitamin B12 deficiency due to pernicious anaemia and resultant atrophic gastritis. Hyperhomocysteinaemia due to vitamin B12 deficiency was identified as a potential contributor to her rec...

Vitamin B12 deficiency-induced megaloblastic anemia in a pediatric patient with autism spectrum disorder with a chronically unbalanced diet
We report the case of an 11-year-old boy with ASD who developed megaloblastic anemia due to vitamin B12 deficiency. He had a prolonged history of selective eating for more than 10 years. His nutritional status on admission was poor, and he had low weight and short stature. His food selectivity was so strong that intervention to expand diet variety was unsuccessful. A developmental-behavioral pediatrician found that the patient had visual dominance and could take some medications when suffering from a minor illness. Nutritional supplements were selected after consultation with a nutritionist. Although compulsory treatment w...
