An Unexpected Cause of Lung Disease Identified After Lung Transplantation
Chest. 2023 Oct;164(4):e111-e115. doi: 10.1016/j.chest.2023.04.041.ABSTRACTA 54-year-old woman with systemic lupus erythematosus with associated interstitial lung disease (ILD) presented to the lung transplant clinic for assessment of candidacy for transplantation. She was initially diagnosed with ILD based on clinical and radiographic features (never underwent lung biopsy). In addition, she had associated mixed group I/III pulmonary arterial hypertension. The patient had no family history of pulmonary disease and had never used tobacco and did not have a history of illicit drug use. She was maintained on systemic immunosu...

Myocardial stretch-induced compliance is abrogated under ischemic conditions and restored by cGMP/PKG-related pathways
Discussion: Our results suggest that ischemia impairs the adaptive myocardial response to acute stretching and that this may be partly prevented by pharmacological manipulation of the cGMP/PKG pathway, namely, with PDE5i. Further studies are warranted to further elucidate the potential of this intervention in the clinical setting of acute myocardial ischemia. (Source: Frontiers in Physiology)

Sildenafil Intoxication Can Be Complicated by Seizures and an Increase in Creatine Kinase
We read with interest the article by Kim et  al. about a 61-year-old man with sildenafil intoxication from taking 1500 mg sildenafil with suicidal intent (1). The intoxication manifested itself clinically with multiple embolic strokes and an increase in creatine kinase (CK), which was interpreted as rhabdomyolysis (1). Symptoms, such as conf usion, drowsiness, palpitations, dysarthria, and dizziness, occurred 1 h after taking sildenafil (1). In the case of multiple embolic strokes, the patient received acetylsalicylic acid plus clopidogrel (1). (Source: The Journal of Emergency Medicine)

Real-World Treatment Patterns Among Patients with Connective Tissue Disorder-Related Pulmonary Arterial Hypertension in the United States: A Retrospective Claims-Based Analysis
CONCLUSIONS: Treatment patterns were generally similar between the CTD + PAH and PAH cohorts, although combination therapy was more frequent in the CTD + PAH cohort. Both cohorts may benefit from broader use of all available PAH treatment classes, including combination therapy. Considering the life-threatening nature of PAH, our findings also highlight the need to address the low persistence rates with PAH therapies regardless of etiology.PMID:37728697 | DOI:10.1007/s12325-023-02658-z (Source: Adv Data)

A real ‐world pharmacovigilance study of FDA adverse event reporting system events for sildenafil
ConclusionsThese findings emphasize the importance of monitoring and understanding the potential risks associated with sildenafil. Further investigation is warranted to validate these associations and address previously unrecognized safety concerns. (Source: Andrology)

Phosphodiesterase-5 (PDE-5) Inhibitors as Therapy for Cerebrovascular Dysfunction in Chronic Traumatic Brain Injury
AbstractMultiple phase III randomized controlled trials (RCTs) for pharmacologic interventions in traumatic brain injury (TBI) have failed despite promising results in experimental models. The heterogeneity of TBI, in terms of pathomechanisms and impacted brain structures, likely contributes to these failures. Biomarkers have been recommended to identify patients with relevant pathology (predictive biomarkers) and confirm target engagement and monitor therapy response (pharmacodynamic biomarkers). Our group focuses on traumatic cerebrovascular injury as an understudied endophenotype of TBI and is validating a predictive an...
