Had a close to brown short incident today anesthetising a 90 yo old lady for an emergency EGD. This after a week of trippy airways and other little challenges leaving me hypertensive. Pt: sweet old lady known to the dept. ASA3, mostly due to AF, htn and light valve insufficiencies, 10x sedations or lma GAs the previous year for ureter stent replacements. In the ED for GI obstruction. Thoracic abdominal CT revealed an enormous hiatus hernia, sphincter closed,ventricle was partially lodged... EGD airways (Source: Student Doctor Network)

To Resign or Not to Resign
I am looking for some advice. I am a practicing OBGYN in a midsize town. I am 2+ years out of residency and I have found myself in a dilemma. Last year was particularly rough surgically. I performed >75 robotic procedures last year and of these, there were 2 ureteral injuries, 2 incidental enterotomies and 4 postop pelvic abscesses. All the pts eventually did well. RP recurrence of urothelial carcinoma
67 yo woman COPD very poor lung fx (FEV 1 < 1.0) on 4L O2 at rest s/p nephroureterectomy 1 year ago for localized transitional cell carcinoma of the renal pelvis with recurrence in the left retroperitoneum on 6 month fu scan after surgery. Med onc thought it was a nodal recurrence (i feel more likely local recurrence) gave her chemo for a few months; the recurrence was about 1.5 cm in size, now its about 2.5, no other dz has developed.
