Hospitalization is a missed opportunity for HIV screening, pre-exposure prophylaxis, and treatment
DISCUSSION: We observed poor provision of HIV testing, PrEP and other HIV services for hospitalized PWID across multiple U.S. medical centers. Future initiatives should focus on providing this group with comprehensive HIV testing and treatment services through a status neutral approach.PMID:38528590 | PMC:PMC10964564 | DOI:10.1186/s13722-024-00451-z (Source: Addiction Science and Clinical Practice)

Factors associated with preoperative delays or cancellations of scheduled non-obstetric surgeries
Prior research has demonstrated racial and socioeconomic disparities in surgical care. This has primarily focused on delays from cancer diagnoses to surgical treatment, such as surgical management of breast and endometrial cancer among non-Hispanic Black patients, and among patients in lower socioeconomic groups. During the COVID pandemic, these barriers were exacerbated as hospitals needed to pause elective surgeries, and prior studies show that, in other specialties, non-English speaking, uninsured patients living in lower incomes were less likely to be scheduled or have longer wait times following the pandemic. Disparities Exist in the Experience of Financial Burden Among Orthopedic Trauma Patients: A Systematic Review
AbstractPurpose of ReviewThere are substantial costs associated with orthopedic injury and management. These costs are likely not experienced equally among patients. At the level of the healthcare and hospital systems, disparities in financial burden and patient demographics have already been identified among orthopedic trauma patients. Accordingly, disparities may also arise at the level of the patient and how they experience the cost of their care. We sought to determine (1) how patient demographics are associated with financial burden/toxicity and (2) if patients experience disproportionate financial burden/toxicity and...

Access to Otolaryngologic Telemedicine Care Across the COVID-19 Pandemic at an Urban Tertiary Hospital System
Conclusion: In an urban tertiary medical center, we found significant differences in sociodemographic characteristics between patients who accessed otolaryngologic care in person versus via telemedicine through different phases of the COVID pandemic, reflecting possible barriers to care associated with telemedicine. Further studies are needed to develop interventions to improve access.PMID:38488168 | DOI:10.1177/01455613241240560 (Source: Ear, Nose and Throat Journal)

Effect of Patient Navigation on Completion of Lung Cancer Screening in Vulnerable Populations
CONCLUSIONS: In this study, lack of order placement was a key reason for incomplete LCS steps. When orders were placed, patients who received navigation had higher rates of completion. Clinical team education and enhanced electronic health record processes to simplify order placement, coupled with patient navigation, may improve LCS in safety-net health care systems.PMID:38489927 | DOI:10.6004/jnccn.2023.7094 (Source: Journal of the National Comprehensive Cancer Network : JNCCN)

Pregnancy Care Utilization, Experiences, and Outcomes Among Undocumented Immigrants in the United States: A Scoping Review
Undocumented immigrants face many barriers in accessing pregnancy care, including language differences, implicit and explicit bias, limited or no insurance coverage, and fear about accessing services. With the national spotlight on maternal health inequities, the current literature on undocumented immigrants during pregnancy requires synthesis. (Source: Womens Health Issues)

Health Care Use Among Patients Retroactively Insured via a Hospital-Based Insurance Linkage Program
ConclusionHospital-based insurance linkage reached three-quarters of uninsured patients and was associated with increased utilization of acute and outpatient health care services. An acute care encounter represents an opportunity to connect patients to insurance, a key step toward improving their health outcomes. (Source: Journal of General Internal Medicine)

Improving Diabetes Management Screening Adherence in a Free Clinic
Uninsured people with diabetes are at-risk for inconsistent disease management and complications that are identifiable through disease management screenings. Electronic point-of-care (POC) tools can improve screening adherence but are not always available in settings with limited resources. This nurse practitioner-led quality improvement project aimed to increase the percentage of free clinic patients with diabetes who were up-to-date with disease management screenings by implementing a paper POC tool and workflow. Preoperative prescription opioid use as an independent predictor of 90-day mortality and adverse events in craniotomy and craniectomy patients
CONCLUSIONS: Preoperative opioid use was independently associated with 90-day postoperative adverse events and mortality. Opioid use increases vulnerability in craniotomy/craniectomy patients and necessitates close monitoring to improve outcomes.PMID:38457788 | DOI:10.3171/2024.1.JNS23615 (Source: Journal of Neurosurgery)
