Synergistic effect of albuminuria on atherosclerosis in patients with primary aldosteronism
CONCLUSION: Concomitant albuminuria in PA was associated with greater arterial stiffness and more substantial improvement after targeted treatment. Both the baseline and the improved extent of PWV increased in correlation with rising urine albumin-creatinine ratio levels, reaching a plateau when the urine albumin-creatinine ratio surpassed 100 mg/g.PMID:38362007 | PMC:PMC10867404 | DOI:10.1177/20406223231210114 (Source: Atherosclerosis)

GSE254742 Sildenafil as a candidate drug for Alzheimer ’s disease: Real-world patient data observation and mechanistic observations from patient-induced pluripotent stem cell-derived neurons.
Contributors : Dhruv Gohel ; Amit K Gupta ; Feixiong ChengSeries Type : Expression profiling by high throughput sequencingOrganism : Homo sapiensAlzheimer ’s disease (AD) is a chronic neurodegenerative disease needing effective therapeutics urgently. Sildenafil, one of the approved phosphodiesterase-5 inhibitors, has been implicated as having potential beneficial effect in AD. We showed that sildenafil usage is associated with reduced likelihood of AD across four new drug compactor cohorts, including bumetanide, furosemide, spironolactone, and nifedipine. For instance, sildenafil usage is associated with a 54% reduced ...

Finerenone: A Novel Drug Discovery for the Treatment of Chronic Kidney Disease
CONCLUSION: The present review covers the complete picture of ADME properties, bioanalytical techniques, clinical trials, toxicity, and possible avenues in this arena. Finerenone is effective compared to other mineralocorticoid receptor-like spironolactone and eplerenone, for the treatment of chronic kidney disease.PMID:38288821 | DOI:10.2174/0115701638283354240103115420 (Source: Current Drug Discovery Technologies)

Healthcare Challenges in the Management of Primary Aldosteronism in Southeast Asia
CONCLUSION: Large populations currently do not have access to the healthcare resources required for the optimal management of PA. Greater efforts are required to improve healthcare access and affordability. Future guideline revisions for PA may need to consider these limitations.PMID:38261997 | DOI:10.1210/clinem/dgae039 (Source: The Journal of Clinical Endocrinology and Metabolism)

Autoimmune Implications in a Patient with Graves' Hyperthyroidism, Pre-eclampsia with Severe Features, and Primary Aldosteronism
Conclusions: This patient manifested severe GD with antibodies undetectable by conventional TSI and TSH receptor assays and accelerated hypertension from PA simultaneously. These conditions were successfully treated separately by spironolactone and thyroidectomy. Autoimmune PA was considered likely given the clinical picture. The diagnosis of PA should be considered in hypertension with GD.PMID:38256430 | PMC:PMC10820415 | DOI:10.3390/medicina60010170 (Source: Medicina (Kaunas))

Transjugular intrahepatic portosystemic shunts for adults with hepatorenal syndrome
CONCLUSIONS: TIPS placement was compared with conventional treatment, with a follow-up of 24 months, in adults with hepatorenal syndrome type 2. Based on two trials with insufficient sample size and trial limitations, we assessed the overall certainty of evidence as low or very low. We are unsure if TIPS may decrease all-cause mortality, serious adverse events, the number of people who did not receive a liver transplant, and the days of hospitalisation because of the very low-certainty evidence. We are unsure if TIPS, compared with conventional treatment, has better effects on overall morbidity (bacterial peritonitis, ence...

Biological Activity of a Coumarin Derivative on Heart Failure Using an Ischemia/Reperfusion Injury Model
Drug Res (Stuttg) DOI: 10.1055/a-2228-4258Heart failure is a health problem worldwide. There are some drugs for it, including digoxin, spironolactone, captopril, and valsartan, but some of these drugs can produce secondary effects, such as arrhythmia, cough, hyperkalemia, hyponatremia and hypotension. The aim of this research was to evaluate the biological activity of coumarin (2H-chromen-2-one) and its derivatives (3BrAcet-C, 3–4Br-Ph-C, 4-CN-7D-C, 4-Me-7-Ph-C and 6Br-3-D-C) against ischemia/reperfusion injury as a therapeutic alternative for heart fa...

Alternative treatment of polycystic ovary syndrome: pre-clinical and clinical basis for using plant-based drugs
The most common cause of infertility and metabolic problems among women of reproductive age is polycystic ovary syndrome (PCOS), a multifaceted disorder. It is an endocrine disorder that occurs in approximately one in seven women. Among these PCOS patients, two thirds will not ovulate on a regular basis and seek treatment for ovulation induction. The symptoms vary in their severity, namely ovulation disorders, excessive androgen levels, or polycystic ovarian morphology. All these symptoms require a therapeutic approach. Many drugs are used to eradicate PCOS symptoms, like metformin, clomiphene citrate, spironolactone, and ...

The Microbiome and Acne: Perspectives for Treatment
AbstractThe skin microbiome consists of the microorganisms populating the human skin.Cutibacterium acnes (C.  acnes, formerly namedPropionibacterium acnes) is recognized as a key factor in acne development, regulating inflammatory and immune pathways. Dysbiosis has been described as the imbalance in skin microbiome homeostasis and may play a role in acne pathogenesis. Microbial interference has been shown to be a contributor to healthy skin homeostasis and staphylococcal strains may exclude acne-associatedC.  acnes phylotypes. In this review we present an update on the skin microbiome in acne and discuss how current acne...

Spironolactone Use And Neurocognitive Outcomes Among Patients With Heart Failure And Reduced Ejection Fraction
Spironolactone may play a role in preventing cognitive impairment and Alzheimer's disease (AD). High plasma aldosterone is associated with increased risk of cognitive impairment in patients with hypertension, and the administration of spironolactone has been shown to attenuate cognitive decline in mouse models of type 2 diabetes mellitus and Alzheimer's disease. These findings suggest that spironolactone may hold promise as a neuroprotective intervention for patients with heart failure (HF), which is known to be associated with cognitive impairment and accelerated cognitive decline. Combination of spironolactone and sitagliptin improves clinical outcomes of outpatients with COVID-19: a prospective cohort study
ConclusionsThe combination of sitagliptin and spironolactone reduced duration of COVID infection and hospital visits better than standard therapeutic approaches in outpatients with COVID-19. The effects of combination of sitagliptin and spironolactone in COVID-19 patients should be further verified in a double-blind, randomized, placebo-controlled trial. The primary role of sebum in the pathophysiology of acne vulgaris and its therapeutic relevance in acne management
CONCLUSIONS: To date, only combined oral contraceptives and oral spironolactone (both limited to female patients), oral isotretinoin and topical clascoterone (cortexolone 17α-propionate) modulate sebum production in patients with AV. A better understanding of mechanisms underlying sebaceous gland changes driving AV development is needed to expand the AV treatment armamentarium.PMID:38146664 | DOI:10.1080/09546634.2023.2296855 (Source: Journal of Dermatological Treatment)
