Which antidepressant is the safest to give with Vyvanse?
Serotonin Syndrome: Both Vyvanse and sertraline can increase serotonin levels in the brain. When taken together, there may be an increased risk of developing a condition called serotonin syndrome, which is a potentially life-threatening condition that can cause symptoms like confusion, hallucination, seizure, extreme changes in blood pressure, increased heart rate, fever, excessive sweating, shivering or shaking, blurred vision, muscle spasm or stiffness, tremor, incoordination... Algorithm for GAD
Wondering of some of the more experienced practitioners here can comment on their general GAD algorithm, or how they conceptualize GAD. I've essentially boiled it down to a 3 level system as such: First line, therapy and medicine (SRI - escitalopram or sertraline) Second line, reiterate they should try therapy. Medication wise class switch to SNRI (duloxetine preferred) Third line, reiterate that at this point medications are less established to work, and therapy is the most likely option... OCD tx after SJS on sertraline?
Does anyone know of papers/guidance on SSRI or TCA use for patient with history of stevens johnson syndrome after starting sertraline? I could only find a couple of case reports in languages I am not fluent in regarding antidepressant rechallenge. Tried memantine monotherapy with limited response, pt hesitant about SGAs and asked to reconsider antidepressants, but I can't locate any very specific discussion about risk of recurrence with various options, just vague 'avoid class' stuff... Sertraline and pregnancy
I have a 28 yr old female patient with MDD, recurrent, moderate and GAD who responded well to sertraline and we increased dose to 150 mg daily to target residual symptoms. She has had some nipple discharge but prolactin levels were normal. She was told by PCP that sertraline may be causing the nipple discharge. She also had a pregnancy test which came back positive. She reports numbness and tingling in her hands and fingers and reports that became more of an issue when sertraline dose was... Sertraline and pregnancy (Source: Student Doctor Network)
