Distribution of Acute and Chronic Kidney Disease across Clinical Phenotypes for Sepsis
Chest. 2024 Mar 8:S0012-3692(24)00296-4. doi: 10.1016/j.chest.2024.03.006. Online ahead of print.ABSTRACTBACKGROUND: Sepsis is the most common cause of Acute Kidney Injury (AKI) in critically ill patients. Four phenotypes (α, β, γ, δ) for sepsis, which have different outcomes and response to treatment, were described using routine clinical data in the electronic health record.RESEARCH QUESTIONS: Do the frequencies of AKI, acute kidney disease (AKD), chronic kidney disease (CKD), and AKI-on-CKD differ by sepsis phenotype?STUDY DESIGN AND METHODS: Secondary analysis of a randomized clinical trial of early resuscitation, ...

Ischaemic Gastropathy: An Under-Recognised cause of Abdominal Pain
We present a case involving a 42-year-old female with no history of vascular insufficiency who developed ischaemic gastropathy following a prolonged stay in the intensive care unit, from septic shock secondary to Escherichia coli bacteraemia due to complicated acute appendicitis. This case underscores the importance of the physician's awareness regarding this rare entity and the necessity to consider it in the differential diagnosis of abdominal pain and haematemesis. Prompt diagnosis and treatment may significantly improve survival outcomes in this less-documented pathology, especially in the younger adult population.LEAR...

Circulating N-lactoyl-amino acids and N-formyl-methionine reflect mitochondrial dysfunction and predict mortality in septic shock
ConclusionFuture studies which validate the measurement of lac-AAs and f-Met in conjunction with lactate could define a sepsis subtype characterized by mitochondrial dysfunction. (Source: Metabolomics)

Posterior reversible encephalopathy syndrome associated with dengue fever induced intrauterine death: A case report
T2-FLAIR hyperintense areas with mild restricted diffusion are seen in both posterior parietal and occipital regions. Similar signal areas are also seen in posterior-inferior aspects of cerebellum. Key Clinical MessageIn recent years dengue fever has become a major health concern specifically due to its diverse presentation and adverse outcome. Progression from mild febrile illness to a severe systemic illness may occur in dengue fever including neurological disorder. Here, we report an unusual and rare case of a 20-year-old mother who developed posterior reversible encephalopathy syndrome (PRES) following dengue fever ind...

Case report: A rare case of coexistence of low-grade appendiceal mucinous neoplasia and goblet cell adenocarcinoma in the appendix
ConclusionsThe coexistence of LAMN and GCA are extremely rare in the appendix and may result from the proliferation of two independent cellular lines. The coexistence of distinct neoplasms poses diagnostic and management challenges. Multidisciplinary team discussion may be essential in the effective management of these patients. Clostridium perfringens-induced massive hemolysis treatment with blood purification to target toxins: a case report
AbstractClostridium perfringens can rarely cause severe systemic infections, usually from an abdominal source, associated with massive hemolysis, which is usually fatal. Hemolytic anemia and acute renal injury resulting from toxin action are critical for the development of multiple organ dysfunction syndrome (MODs), making this condition a real emergency, requiring multispecialty skills and aggressive multimodal therapies. We herein describe a case of septic shock from acute cholecystitis with massive hemolysis caused byC. perfringens in a 55  year-old man that was successfully treated with early blood purification and co...

Variability in the perception of palliative care and end-of-life care among hematology professionals from the same reference center in Bahia, Brazil: A descriptive cross-sectional study
CONCLUSIONS: We found considerable differences in the perception of PC and EOL care among professionals, despite following the same protocols. The study also demonstrated variations between healthcare professionals' beliefs and practices and persistent historical tendencies to prioritize aggressive interventions.PMID:38422243 | DOI:10.1590/1516-3180.2023.0255.R1.29112023 (Source: Sao Paulo Medical Journal)
