Greater Compliance With Early Sepsis Management is Associated With Safer Care and Shorter Hospital Stay
This retrospective, cross-sectional study of U.S. hospitals in Medicare ' s Inpatient Quality Reporting Program aimed to determine whether variation in Sepsis/Septic Shock (Bundle SEP-1) compliance is linked to hospital size and measures of safety and operational efficiency. Two thousand six hundred and fifty-three acute care hospitals in Medicare ' s Hospital Compare online database were included in the study. Relationships between SEP-1 bundle compliance, hospital size, and indices of operational excellence (including Patient Safety Index [PSI-90], average length of stay [ALOS] and readmission rate) were analyzed. SEP-1 ...

Comparison of qSOFA, SIRS, and NEWS scoring systems for diagnosis, mortality, and morbidity of sepsis in emergency department
This study was aimed to compare the quick Sequential Organ Failure Assessment (qSOFA), Systemic Inflammatory Response Syndrome (SIRS), and National Early Warning Score (NEWS) scoring systems for diagnosing sepsis and predicting mortality and morbidity.Patients and Methods: A prospective study was designed. qSOFA, SIRS, and NEWS scores were calculated at the admission. The diagnosis of sepsis was made with SOFA scoring initially. The morbidity and mortality of the patients were identified during follow-up. Also, the sensitivity, specificity, negative predictive value, and positive predictive value of three scoring systems w...

A retrospective analysis of combat injury patterns and prehospital interventions associated with the development of sepsis - Carius BM, Bebarta GE, April MD, Fisher AD, Rizzo J, Ketter P, Wenke JC, Salinas J, Bebarta VS, Schauer SG.
BACKGROUND: Combat injury related wound infections are common. Untreated, these wound infections may progress to sepsis and septic shock leading to increased morbidity and mortality rates. Understanding infectious complications, patterns, progression, and ... (Source: SafetyLit)

Early Physical and Occupational Therapy and Discharge Disposition in Patients with Sepsis...American Congress of Rehabilitation Medicine (ACRM) Annual Conference (Virtual), September 2429, 2021
To explore whether implementing an early mobility protocol conducted by professionals from PT and or occupational therapy (OT) is associated with discharge location for a group of patients with sepsis, septic shock, or severe sepsis. A quasi-experimental study design was used with a non-equivalent post-test design. The data were collected for 3 months. Mid-sized Midwestern hospital. The population of the usual care group were 117 adult patients admitted to any unit with a diagnosis of sepsis, septic shock, or severe sepsis from February 1, 2019, through April 30, 2019. The early rehabilitation group were 96 patients admitt...

Branched-Chain Amino Acids Can Predict Mortality in ICU Sepsis Patients
In conclusion, metabolomics using 1H NMR spectroscop y showed encouraging potential for personalized medicine in sepsis. BCAA was significantly lower in sepsis non-survivors and may be used as early biomarkers for outcome prediction. (Source: Current Awareness Service for Health (CASH))

Nursing Care for the Initial Resuscitation of Severe Sepsis Patients
Sepsis is recognized as a major health care problem worldwide. In the United States, mortality from severe sepsis and septic shock remains a serious health problem; yet, the early recognition of sepsis by nurses reduces can reduce mortality, morbidity, and long-term consequences of sepsis for patients. Improving the knowledge of nurses to first recognize the early signs of sepsis and then how to apply the most up-to-date evidence-based treatments can improve outcomes. Enhanced monitoring includes the use of computerized early warning systems to alert nurses of worrisome clinical patterns and promote the early recognition o...

Adherence to the Severe Sepsis and Septic Shock Early Management Bundle (SEP-1) Does Not Lead to Improved Clinical Outcomes
A longitudinal study from a single healthcare system found that adherence to the Medicare Severe Sepsis and Septic Shock Early Management Bundle resulted in some changes in process measures, but did not lead to improvements in clinical outcomes. (Source: Current Awareness Service for Health (CASH))

First-to-second antibiotic delay and hospital mortality among emergency department patients with suspected sepsis
Conclusions: First-to-second antibiotic delay of greater than one hour was associated with an increased risk of hospital death among patients meeting criteria for septic shock but not all patients with suspected sepsis. Tracking and improving first-to-second antibiotic delays may be considered in septic shock. (Source: Current Awareness Service for Health (CASH))

Improving Care of Skilled Nursing Patients: Implementation of Early Sepsis Recognition
An aging population with underlying health conditions, such as heart disease and diabetes, is at high risk for infections, including pneumonia, influenza, and coronavirus disease 2019 (COVID-19). In particular, the number of individuals in skilled nursing and long-term care facilities is increasing and older adults are at greatest risk. Research reveals these infections can lead to sepsis, septic shock, and death unless detected early through a sepsis screening process. The current quality improvement project demonstrates the capabilities of an early sepsis recognition screening tool in a skilled nursing facility and explo...

Delays in Administration of the Second Antibiotic Dose in Patients With Severe Sepsis and Septic Shock
Conclusions: This single-center study found a 19.3% rate of significant delays in antibiotic second dose administration in patients with severe sepsis and septic shock. This study was not powered to identify differences in outcomes in patients with and without significant second dose delays. Additional large-scale studies are needed to investigate the impact of antibiotic second dose delays on outcomes in patients with sepsis. (Source: Current Awareness Service for Health (CASH))

Antibiotic therapy in sepsis: No next time for a second chance! "
What is known and objective: Sepsis is a life ‐threatening organ dysfunction associated with a high rate of morbidity and mortality. Appropriate antibiotic therapy remains the cornerstone of sepsis and septic shock management. Comment: Although the early initiation of antimicrobial agents in the treatment of sepsis is widely acknowledged, the selection and adjustment to optimal dosage can be equally important. Since significant pathophysiological changes in the critically ill patients lead to altered pharmacokinetics of antibiotics, early consideration of pharmacokinetic/pharmacodynamic (PK/PD) properties is necessary fo...
